Appendix D.2 Fish Shelter Specifications
Fish shelter specifications:
Contractor will provide all the labor and materials necessary to construct and place fish structure in areas designated by the COR. Each structure location on the lake will be marked with a GPS point (in decimal degrees) and provided to the COR. Fish shelters will be constructed of a household 5-gallon bucket with holes drilled 1-1/8” in 4 vertical rows angled down the bucket (16 holes total) as to not overlap the next row. Each hole will have an 8’ length of 1” black water pipe run through it (8 total pipes) to the opposite hole and pulled through to be evenly spaced. Out of the top of the bucket will be 6 more 1” black water pipes 4’ in length evenly spaced to fill gaps. The bucket will then be filled with concrete (approximately 1, 80lbs bag) mixed to manufacturer’s recommended rate to the top of the bucket encasing all pipes. Extreme care must be used when placing the structure in designated areas to ensure the structure is placed vertically where the base of the bucket sits on the bottom of the lake floor. Contractor will place a quantity of 50 fish shelters annually (in accordance with Appendices B & O)

Appendix P Quality Assurance Surveillance Plan
1. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP) - This plan will be used to assure the Government that the work specified under this contract is completed satisfactorily. Payment Analysis will be conducted by using the 100% sampling method. The government reserves the right to alter or change the type of inspection plan at any time, and to make deductions accordingly.
2. 100% INSPECTION METHOD - Surveillance by this method requires that every occurrence of a performed service be monitored. Contract requirements using these evaluations are generally those that occur infrequently, or are essential, or are costly to perform, or at the Government's discretion. These results are then compared to the specific performance standards.
a. Monitoring - Inspection of all services performed under this method will be conducted upon completion of the Contractor's Quality Control Inspection. All services will be inspected and used for evaluating performance.
b. Deductions - The COR may require that all work which fails to provide the desired results be redone or reduce the contract price to equal the reduced value of the service.
The Contractor will ensure that these defects do not recur. If the Contractor fails to rework the rejected areas where the defects are recurring the Government may:
i. Reduce the contract price to equal the reduced value of the service.
ii. Perform the required services, by contract or otherwise, and deduct any costs incurred by the Government that is directly related to the performance of such services from the Contractor's monthly invoice.
iii. Terminate the contract for default. (Refer to Contract Clause 41 for further clarification.)
iv. Deductions - The deductions taken under this method will be based on the following examples:
c. EXAMPLE: a. Contract price for cleaning and inspecting buoys is = $150.00.
d. Number of buoys to be cleaned/inspected is 70.
e. Sample size is 100%, or 70 buoys.
f. Number of defects is 14 buoys.
g. Percentage of Sample defects is .20.
1. (d/c = 14/70 = .20)
h. The deduction from the delivery order invoice for cleaning/inspecting buoys is $30.00.
1. (a X e) = g
2. ($150.00 X .20) = $30.00
3. Should it become necessary for the Quality Assurance inspector to perform re-inspection of defective work that was required to be redone (due to failure of contractor Quality Control
System to locate and cure these deficiencies prior to the QA inspection) there will be a re-inspection/administration charge equal to the Actual Government Cost (AGC) at the Effective Hourly Rate (E.H.R.) applied to the re-inspected bid item. The AGC re-inspection time will start the minute the inspector is called and/or stops his other duties to perform the re-inspection and will end after the inspector returns to his duty site after the inspection. These charges will be deducted from the Contractor's invoice. Deductions for re-inspection will be taken based on the following example:
EXAMPLE:
Re-inspection of 14 buoys at Wappapello Lake. The Government's Quality Assurance Inspector was at the office at the time he was called to re-inspect. His actual time to inspect the buoys and return to his duties at the office was two hours. The QA inspector's Effective Hourly Rate (E.H.R.) is $25.00 per hour.
The re-inspection deduction will be made as follows:
E.H.R. X inspection hours = Deduction $25.00/hour x 2 hours = $50.00 Deducted from Invoice
