Base Access Requirements

A site visit is scheduled for:

· Solicitation#: FA665620B0002

· Project ZQEL 19-0014, Repair Various Sidewalks, Ramps, and Steps

· Location: 3976 King Graves Rd, Vienna OH 44473

· Date: 17 June 2020

· Time: 10:00 AM Eastern (If we receive more than a total of 10 names from all interested parties, we will schedule a 2nd site visit for approximately 11:00 AM on the same day and notify those people to come later).

If you are planning to attend the site visit, please review the vetting process for a badge or pass (see attached – page 2). If any of those criteria are met, you will be denied access to the installation. The Employee Data Sheet (see attached - page 3) must be completed and submitted no later than 8 June 2020 by 1:00 PM Eastern and the names must be in alphabetical order. No exceptions will be given for additional time to submit this information. You must limit the number of employees to 2 per company due to COVID-19 and our current requirements to keep the gathering to 10 people or less. Due to Personally Identifiable Information (PII), please submit the information to SAFE at https://safe.apps.mil/. This is an encrypted website. The information will be submitted to jasmine.corpa@us.af.mil through the website.

DAY OF SITE VISIT:

· Check in at the Visitor’s Center (if the Visitor’s Center is closed, go to the gate) to receive your badge/pass. Please allow a minimum of one (1) hour to receive your badge/pass.

· After receipt of badge/pass, parking is located at Contracting, B504.

· The site visit attendees will meet at the 910th Contracting Office, Building 504. Please arrive no later than 10:00 AM Eastern on 17 June 2020.

· All attendees must adhere to the current requirements for base access due to COVID-19 which include: maintaining 6 feet of physical distancing whenever possible and everyone must wear facial coverings. They will not be permitted on the installation if sick or have had close contact with a positive COVID-19 case within 14 days and shall self-evaluate for illness before traveling to YARS. They should limit their interactions with individuals on base to those they have a work related need to interact with and shall not travel to areas on base unless necessary to complete their site visit.

In order to obtain a DBIDS card a favorable Criminal Background History (CBH) check obtained via the National Crime information Center (NCIC) must be completed for all contractors, subcontractors, and their employees prior to allowing access/entry to Youngstown Air Reserve Station. Individuals who have any of the following negative screening criteria may be denied access to the installation:

1) Inability to verify U.S. citizenship, immigration status, or Social Security Account Number.

2) Evidence of present debarment from entry/access to any Federal/ military installation or facility.

3) Outstanding warrant by federal or civil law enforcement authorities, regardless of offense/violation (i.e., an order to arrest has been issued by a judge).

4) Incarceration for 12 months or longer within the past ten years, regardless of offense/violation, unless release on proof of innocence.

5) Any conviction for espionage, sabotage, treason, terrorism, or murder (convicted of attempted/completed homicide and manslaughter).

6) Individual's name appears on any federal agency's Watch List or Hit List for criminal behavior or terrorist activity.

7) Individual is unable to obtain a favorable National Agency Check (NAC)

8) Registration as a sex offender.

9) Conviction of firearms or explosives violation within the past ten (10) years.

10) Conviction of sexual assault, armed assault/robbery, rape, child molestation, drug possession with intent to sell, drug distribution, or trafficking in humans.

11) 10 or more habitual felony convictions over the past 10 years.
