About this file
This is a Past Performance Questionnaire form for evaluating contractors being considered for Vehicle Barrier Maintenance and Repair services at Joint Base McGuire-Dix-Lakehurst, NJ. The questionnaire uses a 0-5 rating scale (from Unsatisfactory to Exceptional) to evaluate 16 performance criteria including management capabilities, quality control, problem-solving, maintenance execution, and overall performance.

The questionnaire also asks whether any cure/show cause notices were issued and if the respondent would award future contracts to the contractor.
