5. Type of Foundation: ___________________ (CMU block, poured concrete, slab on grade, rubble, frame, brick, steel, wood piling, post)
6. Height of Foundation: ___________________ (slab on grade, joists setting on dirt, 1” to 5’ crawlspace, 5’ to12’ basement)
