RE: Sunset Crater

50,000 Gallon EWT

Pittsburg Tank & Tower Maintenance Co., Inc. September 21, 2017

Photo shows the tank is secured with fencing. We recommend posting a Warn-ing, Tampering With This Facility is a Federal Offense (US code title 42, section 300i-1) sign.

Photo shows the condition of the foundations. AWWA D100-11; 12.7.1: Design of Foundations Height Aboveground states, “The top of the concrete founda-tions shall be a minimum of 6” above the finished grade, unless otherwise speci-fied.” We recommend clearing any dirt, debris and other loose gravel away from the tank foundations, down to a minimum 6” below top of foundations. This should be done by a local excavating company.

Photo shows the condition of the foundations. We recommend removing all dirt, debris and loose gravel from the foundations, repairing any cracks and spalling in the concrete with a commercial non-shrinking grout, grouting around the base of the tank foundation connections to prevent water from entering then sealing the foundations with a sealant.

Photo shows the tank is electrically grounded for lightning protection as required by OSHA 29 CFR 1926 (K) and appears to be in good condition.

Photo shows the condition of the anchor bolts. The structural integrity of the an-chor bolts should be maintained to withstand 100 mph winds blowing from any direction as required by AWWA D100-11; 3.8: Anchorage. We recommend cleaning the area around the anchor bolts, tightening the anchor nuts to specifi-cations, then tack welding on the circumference of the nut-to-base plate connec-tions and tack welding the bolt-to-nut connections for preventive maintenance.

Photo shows the condition of the existing 3” riser drain valve, which is not equipped with a locking device or splash pad. We recommend installing a lock-ing device to prevent unauthorized draining of the tank and a splash pad to di-rect water away from the foundation.

Photo shows the tank name plate.

Photos shows the riser manway is insulated.

RE: Sunset Crater

50,000 Gallon EWT

Tower access ladder in above photos is 16” wide, but is not equipped with anti-skid rungs. Notice the deterioration of the side rail and the paint on the safety climb. OSHA 1926.1053 Ladders states, “Fixed ladders with structural defects-such as broken or missing rungs, cleats or steps, broken or split rails or corrod-ed components- must be withdrawn from service until repaired.” We recommend removing the outer connection plate and weld flat bar re-enforcement to both sides of connection, then installing anti-skid rung covers, replacing the cable type ladder safety device, and posting a Fall Protection Required sign at the base of the ladder.

This work should be performed on an emergency basis.

Photo shows the condition of the windage rods, which are in tune and appear to be properly adjusted.

Photo shows the condition of the strut end connections. The strut ends have been welded and appear to be in good condition.

Photo shows the condition of the riser pipe and bowl connection, which appears to be in good condition.

RE: Sunset Crater

50,000 Gallon EWT

Photos show the condition of the existing 42” high handrail system. OSHA

1910.23 (e)(1) states, “A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42” nominal from upper sur-face of top rail to floor, platform, runway, or ramp level.” We recommend in-stalling an intermediate rail and a swing gate at the opening in the handrail at the junction of the tower access ladder and structural girder.

Photo shows where water is ponding on the balcony floor, causing deterioration of the paint and steel. We recommend drilling additional weep holes in the bal-cony floor to prevent ponding of water.

Photo shows the condition of the 24” primary shell manway. AWWA D100-11;

7.4.4: Shell manholes states, “Two shell manholes shall be provided in the first ring of the tank shell. At least one manhole shall be circular with a minimum di-ameter of 30” (760 mm).” The primary manway requires the following to be in compliance with AWWA D100-11; 7.4.4: Shell manholes and OSHA 1910.146 (c)(2) Confined spaces.

We recommend:

Post Confined Space Entry sign Install maintenance free galvanized steel bolts

Photo shows the condition of the 24” secondary shell manway. AWWA D100- 11; 7.4.4: Shell manholes states, “Two shell manholes shall be provided in the first ring of the tank shell. At least one manhole shall be circular with a minimum diameter of 30” (760 mm).” The secondary manway requires the following to be in compliance with AWWA D100-11; 7.4.4: Shell manholes and OSHA 1910.146 (c)(2) Confined spaces.

We recommend:

Replace 24” manway with 30” manway Post Confined Space Entry sign

Photo shows the condition of the posthead connections. The postheads secure the column legs to the tank shell and support the tank's live and dead load. The postheads have been welded around the circumference of the posthead-to-shell connection for reinforcement and appear to be in good condition.

RE: Sunset Crater

50,000 Gallon EWT

Photos show the condition of the liquid level indicator. We recommend cleaning and lubricating all moving parts on the liquid level indicator for preventative maintenance, then adjusting and calibrating the unit.

Shell-to-roof access ladder in above photo is 16” wide, but is not equipped with anti-skid rungs. Notice the paint on the safety climb. OSHA 1926.1053 Ladders states, “Rungs must be corrugated, knurled, dimpled, coated with skid-resistant material or treated to minimize slipping.” We recommend installing anti-skid rung covers and replacing the cable type ladder safety device.

Photo shows the condition of the tower access standoff platform, which is equipped with an anti-skid floor and a 42” high OSHA compliant handrail sys-tem, complete with an intermediate rail and toeboard. We recommend installing a swing gate.

Photo show the tank roof edge is not equipped with a required handrail system for fall protection. OSHA 1910.23 (c)(1) states, “Every open-sided floor or plat-form 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified).” We recommend installing an OSHA compliant 42” high handrail system around the circumference of the tank roof, complete with intermediate rail, toeboard and a swing gate at the junction of the shell-to-roof access ladder and tank roof.

Photo shows the condition of the existing 24” roof manway. Roof openings on this tank require the following to be in compliance with AWWA D100-11, 5.4.3:

Roof openings and OSHA 1910.146 (c)(2) Confined spaces.

We recommend:

Install 30” secondary roof manway 180° from primary roof manway Post Confined Space Entry signs Install lock on primary manway

Photos show the tank exterior coating system. We recommend pressure wash-ing the tank exterior with biodegradable detergent injection (minimum 3,500 psi at 3.0 gpm) then removing all loose rust and scale with wire brushes and hand scrapers in accordance with SSPC#2 (hand tool cleaning), spot priming and ap-plying one (1) finish coat of acrylic paint.

Photo shows the interior access ladder, which is 16” wide and equipped with an-ti-skid rungs in compliance with AWWA D100-11; 5.4.1: Ladders. We recom-mend installing an OSHA compliant interior access ladder complete with stand-offs every 10' on center and anti-skid rungs, and a cable type ladder safety de-vice at the suggested secondary roof manway

*In cold climates it’s up to the owner’s discretion on placement of internal ladders.

RE: Sunset Crater

50,000 Gallon EWT

Interior bowl access ladders in above photos are 16” wide and equipped with anti-skid rungs as required by OSHA 1926.1053 Ladders and AWWA D100-11;

5.4.2: Ladders. We recommend installing cable type ladder safety climb devic-es on both interior bowl ladders.

Photo shows the condition of the interior roof seams, which appears to be in good condition.

Photo shows a fill pipe on the tank interior. A temperature difference between the water in the top and bottom of a tank, even as little as 1-2 degrees Fahren-heit, is an indication of thermal stratification and the tank water not being com-pletely mixed. Incomplete mixing would result in short-circuiting, and localized increase in water age would develop inside the tank. This typically leads to wa-ter quality problems, such as loss of residual, DBP spikes, HPC spikes, bacteria regrowth, formation of bio-film, changes in pH and dissolved oxygen. We rec-ommend installing a mixing system. Electrical work to be done by others.

Photo shows little to no sediment in the tank interior. We recommend installing a passive cathodic protection system.

Photo shows the riser pipe opening, which is equipped with a safety grill as re-quired by AWWA D100-11; 5.1.1: Safety Grill and OSHA 29 CFR 1910.23 (a)(1) and appears to be in good condition.
