STATEMENT OF WORK

Scope of Work

SCOPE OF WORK

Project Identification: The purpose of this contract is for a contractor, with the appropriate credentials and Licensure to Replace 62 water meters of various sizes park wide. (See attachment for specs) additionally the contractor shall replace 66 backflow preventors at Boulder Beach, NV (See attachment for specs). This project includes a bid option for the replacement of 24 Backflow preventers at Katherine Landing, AZ. Work includes associated mechanical joints needed, parts, fittings, thrust blocking, delineation, flagging, traffic control, trenching, grading, backfill and labor to provide a functioning water distribution system that satisfies the needs of the NPS unit as determined by NPS personnel. The new backflows and meters must be installed one at a time and tested for proper operation to minimize any chance of down time for downstream customers. Switch over to new backflow preventors and meters will be scheduled with and observed by NPS personnel. Disposal of all replaced devices and associated hardware must be removed off site by the contractor. Water Outages: If the instillation requires a water outage to a concession facility or a residential area, the Contractor shall notify the COR 48 hours in advance. The Contractor shall provide the COR with the BPA number, and the length of the proposed outage.

Replace Parkwide Meters New water meters shall have remote reading capability allowing the meter to function in a vault but to be read by park staff from above ground without entering or opening any vaults.

New meters and associated hardware must be NSF 60 & 61 compliant for use with drinking water and UPC (Uniform Pluming Code).

Contractor will supply all new gaskets and associated hardware for the installation of the water meters.

Contractor will check function/operation of new meters, new remote reading system and ensure that there are no leaks or errors and the system functions as designed.

At a minimum 4-meter reading devices (such as handheld wands guns etc) must be provided to the NPS.

All devices must be appropriately sized for their use and in the case of meters they must have the appropriate multiplying factor for their application.

Records must be maintained and conveyed to the NPS to include Serial number, size, Multiplier, Make and Model, Date installed, GPS location for each device, and must be conveyed in a digital format such as excel.

Meters shall be capable of functioning as designed to give accurate readings in low, medium, and high flow situations.

All gaskets hardware accessories etc. shall be new, appropriately sized and supplied by the contractor. The contractor will not reuse any associated parts and pieces from the replaced devices.

New meters must have the following characteristics:

*Meet AWWA Standard C701 and C702 class 2 standards *NSF/ANSI Standard 61, Annex F and G approved *Measurement principles yielding enhanced accuracy and longer life than other meters *One moving part to operate virtually without friction in upper and lower flow ranges *Reduced head loss *Electronic register with no mechanical gearing.

*Configurable LCD display that reads to 9 digits

* Low battery alarm *Vertical and horizontal installations.

*10 year minimum battery life.

PROJECT LOCATIONS:

Boulder Beach, NV developed area.

Katherine Landing, AZ developed area Cottonwood Cove, NV developed area Willow Beach, AZ developed area Las Vegas Bay, NV developed area Callville Bay, NV developed area

Replace and Successfully Test/Certify Backflow Preventers at Boulder Beach, NV and Katherine Landing, AZ Replace 66 backflow preventors at Boulder Beach, NV (See attachment for specs).

With bid option for the replacement of 24 Backflow preventers at Katherine Landing, AZ.

All devices must be testable devices and appropriate for the application in which it is installed.

New backflows must be NSF 60 & 61 compliant for use with drinking water.

Contractor will supply new gaskets with water meters during install.

Contractor is required to submit at minimum a daily progress report, plan for the next day, before and after pics that assist in documenting the installation process and progress.

All Backflow devices must be tested and certified by a certified backflow tester and appropriately tagged with weather and UV resistant tags capable of a lifespan of greater than one year.

All gaskets hardware accessories etc. shall be new, appropriately sized and supplied by the contractor. The contractor will not reuse any associated parts and pieces from the replaced devices.

New devices must be installed per manufactures specification in each discrete application to ensure protection of the distribution system and per ADEQ and NDEP regulatory guidelines respectively.

Records must be maintained and conveyed to the NPS to include Serial number, size, Make and Model, Date installed, GPS location for each device, and tested/certified date and must be conveyed in a digital format such as excel.

All devices must pass certification testing.

New Backflow devices will have the following characteristics:

• Single access cover and modular check construction for ease of maintenance

• Top entry - all internals immediately accessible

• Captured springs for safe maintenance

• Internal relief valve for reduced installation clearances

• Replaceable seats for economical repair

• Bronze body construction for durability 1∕4" – 2"

• Ball valve test cocks — screwdriver slotted 1∕4" – 2"

• Large body passages provides low pressure drop

• Compact, space saving design

• No special tools required for servicing

PROJECT LOCATIONS:

Boulder Beach, NV developed area.

Katherine Landing, AZ developed area (Bid Option)

Award selection and Contract Bid Options The replacement of the Katherine Landing backflow preventers will be presented as a bid option. A best value evaluation will be made concerning all bid packages and at that time the decision to award or not award the Katherine Landing portion of the contract will be made. As such all prospective bidders must provide a price schedule wherein a clear delineation is made between components of the contract (Parkwide Meter replacement, Boulder Beach Backflow Preventer Replacement and Katherine Landing Backflow Preventer Replacement) is made.

PERFORMANCE PERIOD:

This contract will have an initial period of performance of 180 calendar days from time of award.

CONTRACTOR PERSONNEL

The services will be performed by a quoter with the appropriate credentials and licensure.

QUALITY CONTROL - All work shall be performed in accordance with the following:

Industry Standards:

The Uniform Plumbing Code; Recommended Practice for Backflow Prevention and Cross- Connection Control.

The Manual of Cross-Connection Control.

State Administrative Codes:

Arizona Administrative Code (AAC): title 18 - environmental quality, chapter 4 - department of environmental quality safe drinking water, supp- 04-1, article 1 - general requirements, Section R18-4-115 - Backflow Prevention.

Nevada Administrative Code (NAC) and Nevada Revised Statutes (NRS): NAC 445A.67245

PHYSICAL SECURITY

Some locations may be secured and the quoter must make arrangements to access those locations with the assigned COR, 24-hour notice must be given to COR before accessing those locations.

HOURS OF OPERATION/WORK SCHEDULE

Standard hours of operation/work shall be Monday through Friday between the hours of 7:00 AM and 4:00 PM unless alternate hours are approved by the Contracting Officers Representative (COR). The Contractors proposed quote price should be based on working only during standard workdays and hours.

Work Schedule: Upon receiving contract award from the Contracting Officer he/she shall develop a work schedule and submit it to the COR for approval, starting with the orientation meeting. The COR will use this schedule to provide to coordinate with other involved NPS work groups as well as concessionaires.

CONTRACTOR USE OF SITE

General: Limit use of the site during the period of performance to the project area limits as outlined on the attached site maps. Do not disturb portions of the site beyond the project area limits.

Park Entry: The contractor shall furnish a list of personnel with phone contact information.

Vehicles associated with the contractor shall be waived entrance fee upon entering in a marked company vehicle or by providing the names of employees entering in personal vehicles. The entrance fee is waived only for the purpose of reporting directly to a specific work location. Any deviation for recreational purposes is subject to payment of required entrance fees.

Site Access: The contractor shall access the project site as outlined on the attached site maps. Do not drive vehicles off road or access non-project areas without permission from the contracting officer or park superintendent.

Park Regulations: Ensure all personnel (contractors, subcontractors, and vendors) entering or working in the park are familiar with and obey all park regulations. Law enforcement rangers will intervene if they perceive any unsafe or unlawful activity.

Working Hours: Regular work hours shall be limited to daylight hours from 7:00 a.m. until 5:00 p.m. Monday through Friday, excluding government holidays. Work outside regular hours requires approval from the contracting officer.

No Smoking Policy: Smoking is prohibited inside or outside within 25 feet of any buildings in the park. This applies to existing buildings, buildings under construction, and buildings under renovation. Discarding of tobacco products or debris anywhere other than into designated tobacco receptacles is considered littering and subject to fines.

Staging and Storage: Confine storage of equipment and materials to the staging / lay-down / storage areas identified on the attached site maps or as established at the preconstruction meeting.

Parking: The contractor shall park vehicles and heavy equipment in designated areas for contractor use or as established at the preconstruction meeting. Vehicles are not permitted within long-term or overnight parking along roads or in associated overlooks / pull-outs.

Driveways and Entrances: The contractor shall keep driveways and entrances serving the premises clear and available to the government, government employees, and emergency vehicles at all times.

Preservation of Natural Features: Prevent damage to natural surroundings. Restored damaged areas and replace damaged trees, vegetation, and grass at no expense to the government.

Refer also to Section 01 11 40 Work Restrictions and Section 01 50 00 Temporary Facilities & Controls.

CONSERVATION OF UTILITIES

All Contractor personnel shall make every effort to conserve energy when performing

MEETINGS

One Prebid site visit will be scheduled prior to close of solicitation for all interested bidders to attend.

After contract award the Contractor shall attend an orientation meeting to be conducted at the Lake Mead Maintenance Warehouse Building, located at 101 Katzenbach Dr, Boulder City, NV 89005.

Lake Mead personnel will be present to go over park regulations and the specific requirements of the project with the contractor.

TECHNICAL REQUIREMENTS

Contractor is required to submit at minimum a daily progress report, plan for the next day, before and after pics that assist in documenting the installation process and progress.

Work includes associated mechanical joints needed, parts, fittings, thrust blocking, delineation, flagging, traffic control, trenching, grading, backfill and labor to provide a functioning water distribution system that satisfies the needs of the NPS unit as determined by NPS personnel.

The design of this project must be approved by NPS LAKE Utility Office.

The contractor will be responsible for any required permits or compliance needs from the State of Arizona and Nevada. All new utility work will comply with AWWA, NPS and State of Arizona and Nevada regulations. Permits must be stamped by a AZ, NV P.E. when required. Copies of all permit submittals and approvals will be provided to NPS asap.

GENERAL DEFINITIONS

QUALITY ASSURANCE. Actions taken by the Government to assure services meet contract requirements.

QUALITY CONTROL. Those actions taken by a Contractor to control the performance of services to ensure services meet the requirements of the SOW.

TECHNICAL DEFINITIONS

Technical Definitions are described in the BPA manufacturers Operating and Service Manual, and Section 3 of this SOW

GENERAL INFORMATION

The Contractor shall furnish all labor, materials, permits, disposal fees and equipment necessary to complete the project scope of work.

CERTIFICATES/CREDENTIALS

Certifications/Credentials at a minimum should include all pertinent state and federal certifications held by the contracting company and individual technicians who will perform the work onsite.

Capability/Capacity

Capability/Capacity at a minimum should describe the number and credentials of all credentialed employees involved in the all aspects of the project.

GENERAL INFORMATION

The contractor shall have access to the publications listed below to ensure the work is accomplished in accordance with the manufacturer's recommendations and that the work is properly documented prior to submittal to the Government.

PERFORMANCE REQUIREMENTS SUMMARY

The final finished product shall be validation/certification that all BPAs listed on the Price Schedule are fully functioning and have passed all required tests. If a BPA does not pass the initial test, repairs or adjustments shall be made until the BPA can successfully pass the test.

PERFORMANCE EVALUATION

The Performance Evaluation will be based on the spot inspection performed by the COR and the reports submitted by the Contractor.

CONTRACTOR PAYMENT

Payment will take place after the Contractor provides the Government with all the documents listed.

REQUIRED REPORTS

GENERAL INFORMATION

All reports shall be submitted in electronic format, plus one signed original paper copy set.
