Statement of Work

KEAMS CANYON ELEMENTARY SCHOOL

Design build of Two Retaining Walls Keams Canyon, AZ 86034 November 30, 2023

A. GENERAL

1. Purpose:

Keams Canyon Elementary school is seeking the services of a qualified contractor for the design build of two retaining walls located at Keams Canyon Elementary School located at 100 Main Street, Building 48 Keams Canyon, AZ, 86034. The selected Contractor shall be responsible for providing all necessary services, including but not limited to selective design, construction, installation, and supervision, in accordance with the requirements outlined in this statement of work, contract terms, conditions, specifications, and any specific requirements.

It is imperative that the Contractor adheres to all relevant codes, including architectural, civil, structural, mechanical, electrical, plumbing, and other applicable codes as determined by BIA/BIE. All work performed must conform to the prescribed Codes and Standards, ensuring safety, quality, and compliance of the project.

The purpose of this Statement of Work (SOW) is to invite proposals from potential contractors for the work required at this location. It is important to note that this SOW does not constitute an award or a commitment to award the project. Instead, it serves as an initial step in the procurement process, allowing interested parties to submit their proposals for consideration. The final selection and award of the contract will be based on a thorough evaluation of the received proposals and compliance with all relevant procurement procedures and criteria.

2. Project Location:

Keams Canyon Elementary school is situated in the city of Keams Canyon, Arizona.

3. Definitions and Acronyms:

“ADAABAAG” means “Americans with Disabilities Act and Architectural Barriers Act Accessibility Guidelines”.

“A-E” refers to the Architectural and/or Engineering firm. “AHJ” refers to the Authority Having Jurisdiction “ASTM” refers to the American Society for Testing and Materials.

“BIA” refers to the Bureau of Indian Affairs.

“BIE” refers to the Bureau of Indian Education.

“CO” refers to the BIA or BIE Contracting Officer.

“COR” refers to the Contracting Officer’s Representative.

“Design Handbook” refers to the current edition of the BIA School Facilities Design Handbook.

“DOI” refers to the U.S. Department of the Interior.

“DSRM” refers to the “Division of Safety and Risk Management” which is the Authority Having Jurisdiction and located in Albuquerque, NM.

“Plan Review Procedure” refers to the October 10, 2012, Rev. No. 8, “Safety, Health and Accessibility Design Guidelines for Architects, Engineers and Design Firms”.

“SOW” refers to Statement of Work.

B. CODES AND STANDARDS

The Contractor shall provide a multi-disciplined team to furnish the services required to complete the work. Each responsible team member must be knowledgeable of and provide work in compliance with the most current editions of all applicable codes, standards, design handbooks, plan review procedures, and guidelines as adopted by BIA.

• Value Engineering DOI OMB Circular A-131 Where applicable, the Contractor shall adhere to all federal and state statutes pertaining to archaeological, historical, and environmental clearances which may affect this project. Strict adherence to the National Environmental Protection Act (NEPA) and the National Historic Preservation Act (NHPA), Section 106, will be mandatory.

C. SCOPE OF WORK

1. At the discretion of the Contracting Officer, a Pre-Bid Proposal Conference will be scheduled. Refer to the solicitation for the date and time of this meeting.

2. Conduct Site Assessment:

a. Survey and assess the terrain for optimal retaining wall placement.

b. Analyze soil conditions and groundwater levels.

3. Design Phase:

a. Develop detailed engineering plans for the retaining walls.

b. Ensure compliance with ASTM standards and relevant industry regulations.

c. Consider environmental impact and sustainability.

4. Obtain Approvals:

a. Submit designs for necessary approvals.

b. Address any feedback and revise plans as required.

5. Material Selection:

a. Identify and source high-quality materials in compliance with ASTM standards.

b. Ensure durability and longevity of the retaining walls.

6. Construction:

a. Implement proper excavation and foundation preparation.

b. Oversee construction in adherence to design specifications and safety standards.

c. Monitor for quality control throughout the construction process.

7. Drainage Considerations:

a. Incorporate effective drainage solutions to prevent water accumulation behind the walls.

b. Ensure compliance with relevant industry regulations for drainage.

8. Safety Measures:

a. Implement safety protocols during construction to protect workers and school occupants.

b. Install proper signage and barriers to ensure a secure construction site.

9. Quality Assurance:

a. Implement a quality assurance plan to ensure the structural integrity and longevity of the retaining walls.

10. Reporting and Documentation:

a. Regularly update project stakeholders on progress.

b. Maintain detailed documentation of the design and construction phases.

c. Prepare a final report upon project completion.

11. Final Inspection

a. Conduct a thorough inspection of the retaining wall to confirm alignment with the approved design, as well as adherence to all pertinent regulations, codes, and standards.

The contractor will arrange for the following as needed:

a. A secure staging area for storing equipment, materials, tools, and supplies, as the contractor shall have sole liability for said items, and

b. Access to and provision of utilities as needed for design and construction.

c. The contractor shall immediately notify the facility manager and contracting officer of any and all issues that may result in a project delay and/or impact work quality or safety.

d. The contractor shall provide all labor, materials, equipment, accessories, etc., required for the project. All materials and equipment furnished for this job shall be in current production and shall be of quality material. Used, shopworn, demonstrator, prototype, reconditioned, or discontinued equipment or materials are not acceptable.

e. All work will be performed in accordance with current applicable codes. The contractor shall be responsible for obtaining all necessary permits.

f. The contractor shall be responsible for the removal and disposal of parts in accordance with local, state, and federal regulations, and the contractor will provide all the necessary equipment, labor, and materials to safely accomplish said removal and disposal. The contractor shall recover and dispose of the parts per EPA guidelines.

g. All goods and/or services shall comply with applicable OSHA regulations in effect at the time goods are shipped and/or the service is performed. Material Safety Data Sheets

h. Material Safety Data Sheet (MSDS) are required in accordance with applicable regulations. MSDS must be left on site immediately after goods and/or services have been provided.

I. All installations shall be in accordance with the manufacturer’s recommendations.

j. The contractor shall be responsible for removing all construction and packing debris from the work site and keeping a clean, safe work area at all times.

k. If hazardous materials are encountered during execution of the contract, the contractor shall be responsible for removing and disposing said materials in accordance with federal, state, and local statutes and codes. The contractor will provide BIE with appropriate documentation regarding the disposal of said hazardous materials as requested or required.

m. Safety signs, barricades, and/or other materials will be erected by the contractor to warn visitors and staff away from work areas. The safety of visitors, staff, and contractor’s employees shall be a priority and shall be the responsibility of the contractor.

n. The contractor must notify BIE in advance of work that may be disruptive to normal operations and parking lot traffic flow. Any shutdown of service and/or utilities must be approved and scheduled with BIE.

o. The contractor shall be responsible for providing written documentation to BIE for any work that may affect BIE’s warranties or existing equipment. Written documentation must be provided and approved prior to the commencement of work.

p. The contractor shall be held liable for the cost of repair or replacement of structures, utility systems and any other parts of the facility damaged by the contractor’s acts of negligence or lack of full adherence to the requirements of the Scope contained herein. The contractor will be held liable for the cost of repair or replacement of building components and/or vehicle(s) damaged by falling/flying equipment and/or debris.

q. The contractor shall be responsible for the start-up and commissioning of all replaced parts. All systems must be fully functional and operational after installation. If follow-up work is required to correct installation, BIE shall not be charged.

r. Warranties for materials, labor, and equipment will be issued in accordance with industry practices.

s. The contractor shall train key BIE staff in the operation and general maintenance of the perimeter fence and any new equipment. Prior to leaving the job site, the contractor shall provide BIE with all manufacturers' warranty documents upon completion of installation and training.

t. The contractor shall respond to inquiries within twenty-four (24) hours and will service any non-working units on the day of the call during the warranty period. BIE will not be responsible for any additional costs to repair or replace the new retaining walls or parts that are still under warranty; the contractor is responsible for all liability.

u. Unless otherwise specified, the contractor shall unconditionally guarantee the labor and the materials used in performance of this contract within the specified guidelines and recommendations of the manufacturer’s warranty. If any defects or signs of deterioration are noted which in BIE’s opinion are due to faulty workmanship or materials, the contractor shall be notified and shall make the necessary repairs to correct any deficiency in the system at the contractor’s expense.

5. Excavation A. Contractor shall excavate the lines and grades shown on the construction drawings.

Contractor shall use caution not to over-excavate beyond the lines shown, or to disturb the base elevations beyond those shown.

B. Contractor shall verify locations of existing structures and utilities prior to excavation. Contractor shall ensure all surrounding structures are protected from the effects of wall excavation.

6. Foundation Soil Preparation Specifications A. Foundation soil shall be excavated to the lines and grades as shown on the construction drawings, or as directed by the on-site soils engineer.

B. Foundation soil shall be examined by the on-site soils engineer to assure that the actual foundation soil strength meets or exceeds assumed design strength.

C. Over-excavated areas shall be filled with compacted backfill material approved by on-site soils engineer.

D. Contractor shall verify locations of existing structures and utilities prior to excavation. Contractors shall ensure all surrounding structures are protected from the effects of wall excavation.

7. Fill Placement A. Infill soil shall be placed in lifts and compacted.

B. Infill soil shall be placed, spread, and compacted in such a manner that minimizes the development of slack or movement of the geogrid.

C. Only hand-operated compaction equipment shall be allowed within 3 ft (0.9 m) behind the wall. This area shall be defined as the consolidation zone. Compaction in this zone shall begin by running the plate compactor directly on the block and then compacting in parallel paths from the wall face back, until the entire consolidation zone has been compacted. A minimum of two passes of the plate compactor are required with maximum lifts of 8 in. (200 mm).

D. When fill is placed and compaction cannot be defined in terms of Standard Proctor Density, then compaction shall be performed using ordinary compaction process and compacted so that no deformation is observed from the compaction equipment or to the satisfaction of the engineer of record or the site soils engineer.

E. The infill soil shall be compacted to achieve 95% Standard Proctor (ASTM D698). Soil tests of the infill soil shall be submitted to the on-site soils engineer for review and approval prior to the placement of any material. The contractor is responsible for achieving the specified compaction requirements. The on-site soils engineer may direct the contractor to remove, correct or amend any soil found not in compliance with these written specifications.

F. An independent testing firm should be hired by the owner to provide services.

G. Independent firm to keep inspection log and provide written reports at predetermined intervals to the owner.

H. Testing frequency should be set to establish a proper compaction protocol to consistently achieve the minimum compaction requirements set by the design requirements. If full time inspection and testing at 8-inch (20 cm) lifts is not provided, then the following testing frequency should be followed:

a. One test for every 8 inches (20 cm) of vertical fill placed and compacted, for every 25 lineal feet (7.6 m) of retaining wall length, starting on the first course of block.

b. Vary compaction test locations to cover the entire area of reinforced zone; including the area compacted by the hand-operated compaction equipment.

c. Once protocol is deemed acceptable, testing can be conducted randomly at locations and frequencies determined by the on-site soils engineer.

K. Slopes above the wall must be compacted and checked in a similar manner.

8. Infill Soil A. Infill material shall be excavated soil when approved by the on-site soils engineer unless otherwise specified in the drawings. Unsuitable soils for backfill (heavy clays or organic soils) shall not be used in the reinforced soil mass. Fine grained cohesive soils (ф less than 31°) may be used in retaining wall construction, but additional backfilling, compaction and water management efforts are required. Poorly graded sands, expansive clays and/or soils with a plasticity index (PI) greater than 20 or a liquid limit (LL) greater than 40 should not be used in retaining wall construction.

B. The infill soil used must meet or exceed the designed friction angle and description noted on the design cross sections and must be free of debris and consist of one of the following inorganic USCS soil types: GP, GW, SW, SP, GP-GM or SP-SM meeting the following gradation as determined in accordance with ASTM D422.

Where additional fill is required, contractor shall submit sample and specifications to the retaining wall design engineer, or the onsite soils engineer for approval and the approving engineer must certify that the soils proposed for use has properties meeting or exceeding original design standards.

9. Base A. The base material shall be the same as the Wall Rock material or a low permeable granular material. B. Base material shall be placed as shown on the construction drawing. The top of base shall be located to allow bottom wall units to be buried to proper depths as per wall heights and specifications.

C. Base material shall be installed on undisturbed native soils or suitable replacement fills compacted to a minimum of 95% Standard Proctor (ASTM D698).

D. Base shall be compacted at 95% Standard Proctor (ASTM D698) to provide a level hard surface on which to place the first course of blocks. The base shall be constructed to ensure proper wall embedment and the final elevation shown on the plans. Wellgraded sand can be used to smooth the top 1/2 in. (13 mm) on the base material.

E. Base material shall be a 4 in. (100 mm) minimum depth for walls under 4 ft. (1.2 m) and a 6 in. (150 mm) minimum depth for walls over 4 ft. (1.2 m).

F. Base material should be installed to allow for a minimum of one buried block to be extended into the slope to prevent erosion.

10. Deliverables:

a. Submit comprehensive Site Assessment Report.

b. Provide detailed retaining walls Design Drawings and Specifications.

c. Furnish Material List and Cost Estimate.

d. Deliver a clearly outlined Fence Installation Schedule.

e. Present Inspection Reports.

f. Issue a Project Completion Certificate upon satisfactory completion.

Additional Construction Requirements:

1. Contractor shall provide all labor and materials required to complete the work.

2. Contractor shall provide PPE as required.

3. All damage to Keams Canyon elementary school facilities due to construction or staging operations shall be the Contractor’s responsibility to repair.

4. The Contractor shall provide their own dumpster for removed materials and identify all removed materials that are suitable for recycling. In addition, the contractor shall provide their own temporary restroom facilities and hand washing stations on-site.

5. All waste materials shall be removed and disposed of in a manner in accordance with local, State, and Federal regulations.

6. Construction areas will be kept in a safe and orderly state during the project period.

7. The Contractor shall make areas safe during demolition and construction phases using safety tools such as cones, snow fencing, and ribbon tape to keep pedestrian and vehicular traffic safe.

8. The Contractor shall provide a copy of the organization’s safety program to the COR after award of contract.

9. At project close-out, the Contractor shall provide three copies of owner’s manuals, “as built” record drawings, installation instructions, blueprint revisions and other pertinent information (including testing reports) on digital CDs and in hard copy.

D. SUBMITTALS:

A. Shop drawings: Site plan showing layout of the two retaining wall’s location with dimensions, cleared area, elevation of the walls, details of attachments and footings.

B. Certifications: Manufacturers material certifications in compliance with current ASTM specifications.

C. Domestic certifications: Material certifications, made in U.S.A., Buy American Act or Buy America when required.

E. DESIGN/BUILD REQUIREMENTS:

a. Locate, inspect, and determine the capacity of any civil, architectural, structural, mechanical, electrical, and plumbing that will be affected by the construction.

b. The Contractor shall provide A-E services including, but not limited to, construction documents, specifications, and calculations for civil, architectural, structural, mechanical, electrical, and plumbing work as applicable.

c. The Contractor and A-E design team shall meet with the COR and designated Keams Canyon elementary school staff to review design concepts prior to formal submission of construction documents.

d. The Contractor and A-E design team shall locate, inspect, determine suitability, capacity, and all other conditions of the existing site that will be affected by the installation of the two retaining walls.

e. Construction documents including specifications are required. The documents shall be prepared by a qualified Architect and /or Engineer(s) of Record, licensed in the State of Arizona for submittal at 70% (preliminary construction documents), 99% (completed construction documents), and 100% (final construction documents), which are to be reviewed with Keams Canyon elementary school and DSRM. Acceptance and /or approval must be obtained prior to moving on to the next stage of design -refer to the Plan Review Procedure and other guidelines available by request.

F. DSRM SUBMITTAL REQUIREMENTS:

1. Three (3) Sets of Full-Size Drawings

2. Three (3) Sets of Specifications

3. Three (3) Sets of All Other Related Documents (i.e., Amendments, Addenda)

4. Three (3) Sets of Full-Size Shop Drawings and Applicable Related Documents

5. DSRM Design Review Submittal Form (Attachment A)

6. DSRM Letter of Transmittal (Attachment B)

7. All documents submitted to the COR for DSRM review shall contain the State of Arizona registered professional seal of the Designer(s) of Record.

8. The Contractor shall provide electronic copies of all documents.

G. Additional Project Requirements:

1. This work shall be performed by a qualified contractor(s) with experience in the design and construction of retaining walls.

2. The Contractor shall note any historical status, thus is bound by any requirements relating to construction involving a historical facility.

3. Provide a dimensional, boundary, and topographical survey of the retaining wall’s area and proposed locations. Survey to be performed by a qualified professional surveyor licensed in the State of Arizona.

4. Locate all underground utilities in the construction area(s).

5. The Contractor shall engage the services of a qualified, licensed geo-technical engineer to inspect, probe, and evaluate the condition of the underlying subgrade. Provide subsurface investigation, including borings, along with an engineering report containing recommendations for any soil stabilization and subgrade preparation to be included in the project. The A-E design team shall coordinate geotechnical and civil engineering requirements with their design.

6. The Contractor and A-E design team shall verify that the new construction will not negatively impact the existing site.

7. The Contractor shall engage the services of qualified A-E design professionals with experience in retaining walls planning, licensed in the State of Arizona.

8. All materials utilized, and work performed shall follow all applicable codes and standards. The installation shall be done in a quality workmanship-like manner.

9. The Contractor shall provide all labor, materials, and appropriate equipment required to complete the work.

10. All concrete structural foundations shall be constructed with reinforced concrete to be exposed to exterior elements per A-E engineered design. Final concrete mix, configuration, and reinforcing for each application shall be designed by a qualified and licensed A-E professional.

11. Testing and monitoring for soil, base, concrete, reinforcement, asphalt, and welding to be performed by a qualified local third-party testing lab.

12. The concrete mix shall conform to ASTM C94, having a minimum compressive strength of 3,000 PSI at 28 days.

13. The school may be in use during construction. Schedule any deliveries, utility outages, and building closings with the COR.

14. Comply with all Federal, State, and local environmental erosion control requirements during construction operations. Remove installed erosion barriers, fences, etc. after construction is completed. Submit an erosion control plan to COR.

H. TASK DESCRIPTION

16. The contractor shall be responsible for the start-up and commissioning of all installation. The retaining walls must be fully functional and operational after installation. If follow-up work is required to correct installation, BIE shall not be charged.

17. Warranties for materials, labor, and equipment will be issued in accordance with industry practices.

18. The contractor shall train key BIE staff in the operation and general maintenance of all new equipment. Prior to leaving the job site, the contractor shall provide BIE with all manufacturers' warranty documents upon completion of installation and training.

19. The contractor shall respond to inquiries within twenty-four (24) hours and will service any non-working units on the day of the call during the warranty period. BIE will not be responsible for any additional costs to repair or replace new equipment or parts that are still under warranty; the contractor is responsible for all liability.

20. Unless otherwise specified, the contractor shall unconditionally guarantee the labor and the materials used in performance of this contract within the specified guidelines and recommendations of the manufacturer’s warranty. If any defects or signs of deterioration are noted which in BIE’s opinion are due to faulty workmanship or materials, the contractor shall be notified and shall make the necessary repairs to correct any deficiency in the system at the contractor’s expense.

I. DELIVERABLES

1. The Contractor shall review the two retaining wall’s locations, dimensions, details, markings, samples, product data, and other submittals with the COR and facility manager. Submittals shall contain data regarding components, details, and materials that will be used in the project.

2. A copy of the survey, civil drawings, architectural, mechanical /electrical, and structural drawings prepared by and bearing an Arizona Professional seal shall be provided to the COR for the applicable related tasks.

3. The Contractor shall submit an electrical and data /fiberoptic line location plan to the COR.

4. Submit all inspection reports to the COR.

5. The Contractor shall perform its own final inspection of the installation with appropriate checklists and submit this to the COR before the government’s final approval.

6. The Contractor’s inspection checklist report shall include how noted items have been addressed and are to be submitted to the CO prior to securing the government’s final approval.

7. The Contractor shall provide all closed out documents, including but not limited to, warranties, operation, maintenance information, etc. as required by the contract.

J. PERIOD OF PERFORMANCE

The delivery should be completed within sixty (60) calendar days after the contract is awarded, with substantial completion.

K. BACKGROUND INVESTIGATIONS

The Contractor shall be required to submit to a background investigation. The background check process will include, but may not be limited to, filling out hard copy forms, online forms, fingerprinting, and submitting documentation in a timely manner. The contractor shall not be paid for any time associated with this process outside this contract but should include estimated costs in their proposal to meet this requirement. It can take 2-5 weeks for clearance, so it is imperative to start this process as soon as possible.

Typically, the Project Manager and the alternate is to submit paperwork (daily, payroll, testing, and weekly reports). The “Notice to Proceed” will not be issued until the background checks have cleared.
