Services shall be performed during normal business hours (0800–1630 Monday–Friday) excluding federal holidays, with no separate shipping charges permitted. The contractor is responsible for site cleanup and proper disposal of all materials upon job completion. No government-furnished property is provided; the contractor supplies all parts and labor. Hazardous materials data sheets must be submitted prior to contract award if applicable.

Performance Work Statement

For Inspection/Servicing/Repair of Carson City District Fire Extinguishers

1 DESCRIPTION OF SERVICES: Provide all necessary labor, equipment, supplies, mileage, and materials necessary:

• Annually inspect, service, repair, and recharge, all fire extinguishers during the Annual Service of the Fire Extinguishers. (Base Year plus 4 Option Years)

1.1 Period Of Performance: The contractor/vendor shall perform the required services within 60 days of the beginning of the period of performance. This contract will have an awarded base year and 4 option years. Each year service shall be scheduled and started within 60 days beginning of the period of performance and completed within 30 days all places of performance listed below. Below is an estimated schedule.

1.2 Places Of Performance:

• Carson City District Office, 5665 Morgan Mill Road, Carson City NV. 89701

• Palomino Valley Fire Station, 5605 Grass Valley Road, Reno, NV. 89510

• Stead Airtanker Base, 14600 Maryland St. Reno, NV 89423

• Silver State Hot Shot Operations Center, 3250 Hobo Hot Springs Road, Minden, NV. 89423

• Cold Springs Fire Station, 3405 White Lake Parkway, Reno, NV 89508

1.3 Specific Requirements: The contractor shall perform the following tasks:

• The contractor must be licensed in the State of Nevada under NRS Chapter 477 and comply with NFPA 10, NFPA 25 and other Federal, State, or local requirements.

• Inspect handles, safety pin, operating handle, pressure gauge, cylinder, syphon tube, discharge hose, and discharge nozzle for damage, excessive wear, or defects that would impair the function and use of the fire extinguisher using NFPA and NRS Chapter 477 standards.

• Disassemble fire extinguisher to inspect internal components.

• Replace and/or recharge the extinguishing agent if necessary.

• Any required Hydro-testing fire extinguishers based on NFPA 10 requirements will be evaluated on a case-by-case basis.

1.3.1 Oversight of Subcontractor:

The prime contractor shall provide on-site supervision or designate a qualified representative responsible to oversee subcontractor performance. The Government will not provide day-to-day direction or technical guidance to subcontractors, The qualified representative designated by the prime vendor shall not be an employee of the subcontractor. All subcontractors and qualified representatives shall report directly to the prime contractor and not to the Government. Failure to perform in accordance with Performance Work Statement (PWS), to include but not limited to: deficiencies as a result of subcontractor mismanagement or nonperformance, shall remain the sole responsibility of the prime contractor.

Number of Fire Extinguishers and location: The number, type and location of Fire Extinguisher are listed on attached Spreadsheet. Although not all inclusive, however, within 2% plus or minus of total.

1.3.2 Confirming total count of Fire Extinguishers

The contractor shall submit a spreadsheet listing the total number of fire extinguishers serviced. The spreadsheet shall include the location where each extinguisher was serviced, the type/classification of fire extinguisher (A, B, C, etc.), and the extinguisher size in pounds. The contractor shall also annotate any fire extinguisher that was not serviced due to requiring hydrostatic testing. The contracting officer representative shall receive a copy of this spreadsheet for the contract file.

1.4 Location of Facilities

The facilities listed below require annual inspections of Fire Extinguishers.

2 GENERAL INFORMATION:

This is an estimate of the total number and type of fire extinguishers is listed in the attached bid schedule. This estimate is within 2 percent plus or minus. Any fire extinguisher that is deemed un-repairable will be placed out of service. Any purchase of new fire will be outside of this contract and will be evaluated on a case-by-case basis.

2.1 Shipping and Delivery:

Shipping and/or delivery will not be charged separately; any associated costs must be included in the total price.

2.2 Hazardous Materials:

The Contractor is required to submit hazardous materials data whenever the supplies being acquired are identified as hazardous materials. The latest version of Federal Standard No. 313 (Material Safety Data Sheet, Preparation and Submission of) includes criteria for identification of hazardous materials. Hazardous material data (Material Safety Data Sheets (MSDS)) shall be submitted by the Contractor prior to contract award if hazardous materials are expected to be used during contract performance.

2.3 Schedule: The Contractor shall perform services during normal duty hours between 0800 – 1630 Monday- Friday. The Contractor shall not schedule performance of services during Federal holidays as listed below, if the holiday falls on a Saturday, it is observed on Friday. The Contractor shall not work when Carson City District Office is officially closed by the District Manager or their acting, or under an emergency, or special event service.

2.4 Site Clean-Up and Restoration:

The contractor shall ensure the work site areas and around the work site are free of any debris and materials upon completion of work each day, as well as at the completion of the job. The Contractor shall remove and properly dispose of all materials and equipment at the completion of the job.

3 GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES:

No Government furnish property. Contractor will provide all parts and labor.
