About this file

This is a Price Sheet (Attachment A) for Request for Bid #PA180734 issued by the State of Wisconsin Department of Administration on behalf of the Department of Public Instruction for the processing of USDA Bulk Cheese, Natural American FDB Barrel (USDA Foods Material ID Code #110242) into Macaroni and Cheese with Whole Grain Pasta. The finished product will be distributed to all Wisconsin schools participating in the National School Lunch Program (NSLP) and eligible participants in the Summer Food Service Program, including both public and private schools, residential child care institutions, and other USDA food assistance program participants. The contract term runs from July 1, 2018 to June 30, 2019. The State intends to award the contract based on the lowest Total Price per Serving for a product that passes student taste tests.

Bidders must submit pricing on the fee-for-service processor value-pass-through method, which includes the processor's costs for ingredients other than USDA Foods, labor, packaging, overhead, and conversion costs. The pricing sheet requires bidders to enter a Bid Price per Case (Column A) and Pounds per Case (Column B), with the USDA Food Value per Case (Column C) automatically calculated at $1.6929 per pound based on the Average Cheese Prices File for School Year 2018. Bidders must also specify the number of Servings per Case (Column D), where each serving provides a 2-ounce meat/meat alternate and minimum 0.75-ounce equivalent grains to meet Child Nutrition Meal Pattern requirements. The Total Price per Serving (Column E) is electronically calculated to four decimal places using the formula (Column A + Column C) divided by Column D. All bid prices must include costs to palletize and deliver FOB Destination, Prepaid and Allowed, to State warehouse sites, with pricing stated in U.S. dollars. Bidders must also provide the commercial equivalent product code and estimated case product yield per truckload of USDA product for information purposes.
