About this file
This Quality Assurance Surveillance Plan (QASP) outlines the government's methodology for monitoring and evaluating a septic pump-out services contract at Howard A. Hanson (HAHD). The document specifies two primary performance requirements: 1) completely pumping out septic tanks and disposing of waste offsite, with zero deviation from standards, and 2) providing accurate invoices that comply with contract provisions. The U.S. Army Corps of Engineers (USACE) will conduct surveillance through 100% inspection methods, with the Contracting Officer (KO) Scotty Britt and Quality Assurance Representatives (QARs) Warren Douglas and Steven Bjerk responsible for monitoring contractor performance.
The QASP establishes a rating system to evaluate contractor performance, with potential ratings of Exceptional (significantly exceeds requirements), Satisfactory (meets requirements), or Unsatisfactory (fails to meet requirements). The document emphasizes that the contractor is responsible for quality control, while the government will objectively assess performance. Unacceptable performance will be documented and communicated to the contractor, with potential consequences including payment deductions. The plan is considered a "living document" that can be reviewed and revised in coordination with the contractor to ensure it remains a valid and useful monitoring tool.

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations may occur.
This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Updates shall ensure that the QASP remains a valid, useful, and enforceable document. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
