About this file

This is a physical location certification form (VR3442B) from the Texas Workforce Commission Vocational Rehabilitation Services division, required for entities applying to provide Orientation and Mobility Services to VR customers throughout Texas. The form must be completed for each physical location where the service provider will deliver goods, services, or conduct in-person customer meetings. Applicants must provide entity information including legal name, doing business as name, and employer identification number, along with complete physical location details including street address, city, county, state, and ZIP code. The form requires submission of occupancy certificates or permits, building permits, and fire inspection reports obtained from landlords or local fire departments, with documentation of collection attempts required if these documents cannot be obtained.

The form establishes mandatory safety and accessibility standards that each physical location must meet prior to service delivery. Safety requirements include working smoke detectors with visible and audible signals, non-expired fire extinguishers in accessible locations, posted fire escape route diagrams, evacuation plans for customers requiring physical assistance, emergency medical services arrangements, accessible aisles and work zones, hazardous materials security plans, and equipment safety protocols. Accessibility standards require handicapped parking, accessible exterior routes, accessible interior spaces, and accessible bathrooms, with accommodations mandated when minimum standards do not meet individual customer needs. The legally authorized representative must certify the accuracy of all information, compliance with ADA and safety standards, and commit to reporting all safety incidents through the VR3446 Incident Report form to the assigned Quality Assurance Specialist or Regional Program Support Specialist. The form must be updated whenever information changes.
