INSTRUCTIONS:
| (1) Show the prime Offeror's established WYE (i.e. number of productive hours per year). |
| (2) Identify the proposed labor categories for the prime Offeror for Administration and Management, along with the proposed hours, direct labor rate, and direct labor cost for each category for each year. Add or subtract rows to accommodate the number of proposed labor categories. Note: Offerors shall not propose labor costs which are classified as indirect costs in their established accounting system. |
(3) The table above for CLIN 1 has already been populated with the labor categories shown in Form 3. The table shall show all of the prepopulated Form 3 labor categories and the proposed hours for each of these labor categories given the Offeror's established productive WYE and the WYEs listed in Form 3. Also show the direct labor rate and direct labor cost for each category for each year.
(4) Show the total proposed direct labor cost and total proposed hours for all labor categories combined, for each year, and for all years combined.
(5) Show the indirect costs applicable to direct labor (e.g. fringe, labor overhead, G&A) for each year and for all years combined, along with the rate and allocation base for each indirect cost. Tailor Form 2 (e.g., adding and/or deleting row(s)) according to your established accounting policies and practices and classify and include or exclude indirect rates accordingly. The indirect cost allocation bases proposed above shall be in accordance with your established accounting policies and practices.
(6) Show the prime Offeror's total burdened labor cost (excluding fee) (i.e. the sum of the costs in items (4) and (5) just described).
| (7) Offerors shall propose the ODC amounts listed in the table. Offerors may propose ODCs in addition to those specified by the Government (see Section L.19(k)). |
| (8) If applicable, show the total proposed subcontract cost for each year and for all years combined. |
(9) Show the prime Offeror's indirect costs applicable to ODCs and subs for each year and for all years combined, along with the proposed rate(s).
(10) Show the total cost (i.e. the sum of the costs described in items (6), (7), (8), & (9)) for each year and for all years combined.
(11) Show the proposed fixed fee rate and the proposed fixed fee for each year and for all years combined.
(12) Show the total proposed CPFF for each year and for all years combined.

INSTRUCTIONS:
(1) All labor categories / rates proposed in the IDIQ Schedule of Rates (Form 5) shall be included in this Form 6. Form 6 shall identify prime Offeror and subcontractor rates separately for the purpose of proposal evaluation. If any labor category is proposed to be performed by both the prime Offeror and a subcontractor (including any proposed composite rate(s) proposed in Form 5, if applicable), add row(s)/line(s) to that labor category within Form 6 and list the labor rates on multiple rows (e.g. one with the prime Offeror’s rates and one with the subcontractor's burdened rate). Do not combine or reflect Offeror and subcontractor rates as composites in Form 6.

This shall be accomplished by the prime and all subcontractor(s) completing and submitting Forms 5 & 6. Further, if a proposed burdened rate in the IDIQ Schedule (Form 5) is a composite, the prime Offeror shall explain how the composite rate is calculated (e.g. the percent weighting applied to each component of the composite rate). If the rate is for a sub, show the name of the sub. Likewise, if the rate is a composite, show the name(s) of the sub(s) used in calculating the composite. |
All subcontract labor categories must include profit/fee in their fully-burdened labor rates, if profit/fee is proposed. All rates that are solely for subcontractors need only be fully-burdened thru all of the subs' applicable indirect and profit/fee burdens, as well as the prime Offeror's applicable indirect burdens on subcontract costs. Offerors shall identify and explain the basis for any prime Offeror burden(s) on subcontract rates. If prime indirect burdens are applied to the subs' rates, the prime Offeror shall state the rate(s) used for such burden(s)). Any such burden(s) shall be consistent with the Offeror's established accounting policy and procedures. Subcontractors' proprietary data, such as direct labor rates, indirect rates, and fringe benefits, need not be provided in this prime Offeror’s Form 6. However, such data, if not provided in the prime Offeror's proposal, shall be submitted by each subcontractor in its own proposal and shall be submitted directly to NASA no later than the date and time specified in the instructions for receipt of offers for this solicitation.
(6) The prime Offeror shall show the build-up of the burdened rate for each labor category for each contract year (i.e. the direct labor rate and all applicable indirect rates for each category). The allocation bases used to calculate the total burdened rates shall be consistent with the indirect cost allocation bases identified in Forms 2 and 7. Explain how each element of the burdened rate is determined. Offerors shall tailor Form 6 (e.g., adding and/or deleting column(s)) according to their established accounting policies and practices and shall classify and include or exclude indirect rates accordingly.
(7) Each Offeror shall identify its rate of contribution to Health & Welfare (H&W) for each labor category, expressed as dollars per hour worked.
