About this file
This document is a Qualified High Deductible Health Plan design and benefits summary for the Schertz-Cibolo-Universal City Independent School District, effective from September 1, 2024, and administered by Aetna Life Insurance Company as a self-funded plan. The plan provides health insurance coverage for employees and their dependents, with in-network benefits including a $5,000 individual and $10,000 family deductible, 30% member coinsurance, and an out-of-pocket maximum of $6,550 per individual and $13,100 per family. Preventive care services such as routine physical exams, immunizations, well-child visits, and screenings are covered at 100% with no deductible.
The plan offers comprehensive coverage across various medical services, including physician services, diagnostic procedures, emergency medical care, hospital care, mental health services, substance abuse treatment, therapy services, and family planning. Notably, the plan does not cover certain services like cosmetic surgery, dental care, hearing aids, home births, infertility treatments, and weight control services. Dependents are eligible from birth to age 26, regardless of student status, and the plan is provided by Aetna Health Inc. with potential variations in features and availability based on location and group size. The document emphasizes that health benefits contain exclusions and limitations, and not all health services are covered.
