About this file

This is an Account Authorization Request form (FRM-0496) used for requesting computer system access at Nevada National Security Site (NNSS) facilities. The form is specific to Department of Energy/National Nuclear Security Administration (DOE/NNSA) information systems and network resources.

The form consists of two parts: Page 1 covers basic user information, security questions, and computer access details for all users, while Page 2 is specifically required for third-party access (subcontractors, consultants, vendors, or business partners). Key requirements include mandatory cyber security training completion (1S00W180), restrictions on non-DOE/NNSA devices, prohibition of external storage devices, and security scanning provisions. The form emphasizes that all system access is monitored and requires acknowledgment that use is restricted to official work only. Access requests must be submitted encrypted via email to IAM@nv.doe.gov for MSTS employees or CIRT@nv.doe.gov for subcontractors.
