About this file

This is a Safety Documents attachment for the Austin Independent School District (AISD) Request for Proposals regarding custom serving line upgrades at Akins and Bowie High School kitchens in Texas. The document outlines mandatory safety record submission requirements that all bidders must fulfill as part of their proposal response. Vendors are required to submit OSHA 300 Logs and 300A Form Summary Logs for the last three completed calendar years, with the 300A Summary Form required for every year even if no recordable injuries occurred. Additionally, bidders must provide a loss run (statement of claims) from their workers' compensation insurance carrier covering the most recently completed policy year, printed no more than 60 calendar days before the bid due date. The attachment specifies that insurance carrier or agency names must be clearly legible on all documents, and claimant names may be redacted.

Beyond the loss run requirement, vendors must submit a loss ratio document from their workers' compensation insurance carrier corresponding to the most recent completed policy year, also printed within 60 calendar days of the bid deadline. Bidders are also required to provide their current experience modifier from their workers' compensation insurance premiums, obtained from their insurance carrier, insurance agency, or rating agency with the applicable year clearly indicated and printed within the same 60-day timeframe. All loss ratio and experience modifier documents must originate from official insurance carriers or agencies rather than being internally prepared. If no losses are recorded on the loss run, a separate zero percent loss ratio document is not required. These safety documentation requirements must be met for bid consideration.
