About this file

This is a Request for Proposal Introduction document from Fulton County School System (FCS) in Georgia seeking qualified medical service providers to deliver post-offer, annual, and fitness-for-duty physical examination services for school bus drivers in compliance with Georgia Department of Education standards. FCS operates two Transportation Department facilities—one in Alpharetta serving approximately 600 employees and one in Fairburn serving approximately 500 employees—where on-site annual physicals will be provided during 6:30 a.m. to 3:30 p.m. Monday through Friday. Off-site services will be available during standard work hours of 8:30 a.m. to 4:30 p.m. Monday through Friday, with advance notice requirements of 72 hours for annual physicals and 4 hours for post-offer and fitness-for-duty physicals. The selected contractor must provide comprehensive physical examination services including medical and occupational history review, complete physical examination, hearing and vision testing, urinalysis, tuberculosis skin testing and chest x-rays when clinically advisable, and written reports on the state-mandated DE Form 0514 (Revised June 27, 2023).

All physical examinations must be conducted by licensed physicians, with routine measurements and laboratory work performed by nurses or appropriately licensed technicians. The contractor must maintain confidential records, employ experienced and knowledgeable personnel, and provide backup equipment and staff to ensure prompt scheduling. On-site annual physicals will be the primary service delivered at FCS facilities, while post-offer and fitness-for-duty examinations, certain supplemental testing requiring specialized equipment, and off-site audiometric screening will be conducted at logistically convenient off-site medical facilities. The contractor must obtain prior approval from FCS before charging for any services, and FCS will only accept charges for approved work.

REQUEST FOR PROPOSAL INTRODUCTION

1. PURPOSE

FCS is seeking proposals from medical service providers qualified to provide post-offer and on-site annual physical examination services that are in total compliance with applicable Georgia Department of Education standards for school bus drivers. The successful proposer would provide on-site services to FCS’ Transportation Department facilities located at 410 South Main Street, Alpharetta, Georgia 30009 (Approximately 600 employees) and 107 Heath Street, Fairburn, Georgia 30213 (Approximately 500 employees).

2. BACKGROUND

FCS is one of the most unique school systems in the nation. Though not Georgia’s largest school system in terms of student enrollment, it is however; one of the largest systems in geographic area. From its southern end in the City of Chattahoochee Hills to its northernmost tip in John’s Creek, the county is more than 70 miles long. The result is that the Fulton County School System is physically bisected by the City of Atlanta and its school system. While this wedge often leads to references of North Fulton and South Fulton, there is only one Fulton County School System.

Currently, FCS out-sources their bus driver’s physical examination requirements. A copy of the “Post-Offer and Annual Physical Examination for School Bus Drivers Form” is provided in this solicitation.
