About this file
This is an Insurance Requirements document issued by the City of Lewisville, Texas, applicable to the Annual Contract for Chemical Analysis and other general service contracts. The document establishes mandatory insurance coverage standards that vendors must maintain throughout the contract duration to protect against claims for injuries to persons or damages to property arising from contract performance. The insurance requirements apply to a broad range of service categories including janitorial services, maintenance agreements, electrical work, plumbing, welding, and similar service contracts.
Vendors must procure Commercial General Liability coverage with minimum limits of $500,000 per occurrence and $1,000,000 aggregate, Workers' Compensation insurance as required by Texas Labor Code with Employer's Liability minimum limits of $500,000 per injury and per occurrence, and Automobile Liability coverage of $500,000 Combined Single Limit if vehicles are used under the contract. All policies must be written on an occurrence form rather than claims-made form and must name the City as an additional insured with primary and non-contributory coverage. The vendor's insurance must include a waiver of subrogation against the City and provide thirty days' written notice prior to cancellation or material change. Insurers must maintain an A.M. Best rating of no less than A-:VI or Standard and Poors rating of A or better. Vendors must provide certificates of insurance on forms approved by the Texas Department of Insurance, and the City reserves the right to request complete certified copies of all required policies. The vendor agrees to defend, indemnify, and hold the City harmless against all claims arising from the vendor's breach, negligence, or intentional tort, with liability apportioned according to Texas law and excluding indemnification for the City's sole negligence.
