About this file

This is a Cost Proposal Worksheet submittal form for the Utah Department of Transportation's Request for Proposals for a Toll Back Office System and Customer Service Center Support. The RFP solicitation seeks a single contract award from qualified firms to provide comprehensive toll back office system services and customer service center support operations. The initial contract term is five years with options for up to five additional one-year extensions, creating a potential ten-year contract period. The worksheet requires contractors to provide detailed cost proposals across multiple service categories including design and implementation, operations and maintenance, monthly service fees based on active account levels, per-item costs for transponders and statements, end-of-contract transition costs, and optional feature pricing.

The cost proposal must be submitted with pricing for design and implementation costs (capped at 25 percent of total proposal cost), annual maintenance costs for each of the ten contract years (with maintenance costs in years 6-10 not varying more than 25 percent from years 1-5), monthly service fees per active account across three tiered account levels (with fees varying no more than 20 percent between levels within each contract year), and per-item costs for transponder shipments, generated and emailed statements, printing and handling, notifications, and supplies with a 5 percent escalation rate specified for years 2, 5, and 8. The worksheet also requires loaded hourly labor rates for nine staff positions including customer service representatives, software developers, and system administrators, which will be used for scope changes involving labor. Additional optional features may be proposed with separate pricing, including fleet account management, chat capability, mobile app development, trip mapping functionality, and customer service representative co-location at the UDOT facility. All formulas and pricing calculations are the responsibility of the contractor to ensure accuracy.
