PURPOSE: To amend RFP 75N94022R00003, entitled, “International and Domestic Pediatric and Maternal HIV and Other High Priority Infectious Diseases Data Coordinating Center.” This amendment also answers questions received regarding the
RFP.

Except as provided herein, all terms and conditions of the document referenced in item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

A. As a result of this amendment, RFP No. 75N94022R00003, is revised as follows:
1. Section L, item 2.a.16, page 98, is revised to read as follows:
16. Past Performance Information
a. Offerors shall submit the following information as part of their Business proposal.
A list of the last 4 contracts that have either been awarded, are currently being performed or have been completed within the last five years that are similar in nature to the solicitation workscope. Contracts listed may include those entered into by the Federal Government, agencies of state and local governments and commercial concerns. Offerors may also submit past performance information regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. For the purposes of this solicitation, a "major subcontract" is defined as $750,000 or more.
Include the following information for each contract or subcontract listed:
1. Name of Contracting Organization
2. Contract Number (for subcontracts, provide the prime contract number and the subcontract number)
3. Contract Type
4. Total Contract Value
5. Description of Requirement
6. Contracting Officer’s Name and Telephone Number
7. Program Manager’s Name and Telephone Number
8. North American Industry Classification System (NAICS) Code
9. The offeror may provide information on problems encountered on the identified contracts and the offeror's corrective actions.
b . The Government is not required to contact all references provided by the offeror. Also, references other than those identified by the offeror may be contacted by the Government to obtain additional information that will be used in the evaluation of the offeror's past performance.
2. Attachment 1 – PACKAGING AND DELIVERY OF PROPOSAL, subparagraph B. Page limitations, is revised to read as follows:

Parts 2 through 4 - (separate parts of the Technical Proposal) shall address Task Orders Nos. 1, 2, and 3 and shall not exceed 20 single spaced pages each. PROPOSAL PAGE LIMITS exclude resumes. Also excluded from the page limits are: Summary of Related Activities, Table of Contents, Proposal Cover Sheet, Technical Proposal Direct Cost Summary Sheet, Resumes, Bibliography/ References, Human Subjects Separate File, Section 508 Template. The Human Subjects Separate File in the Technical Proposal does not have a page limit. The Data Sharing Plan may be in Summary Form in the 30-page technical proposal and the Summary is not excluded from the page limits. If an offeror elects to provide a more expanded data sharing plan, the expanded plan can be an Appendix and is excluded from the page limits.
Pages that exceed the page limit requirements will not be read, evaluated, or considered for review. Issues that need to be addressed in Task Order 1, Task 2 and Task 3, which have already been detailed in Technical Proposal in response to the IDIQ Contract Statement of Work, should be referenced in the second, third, and fourth parts of your technical proposal rather than repeating passages.

3. Attachment 9 – Additional Proposal Information and Instructions, the chart under Preparation of the Technical and Business Proposal is replaced in its entirety with the following:

B. This Amendment provides answers to questions received regarding this RFP. Please note that individual questions may have been combined or reworded. The responses are provided for information only as guidance to potential offerors and do not supersede any terms of the RFP, and do not revise or become part of this RFP.
1. Question: On page 94, RFP Section L.2.a.3, for the Form NIH 2043 - Proposal Summary and Data Record, is this form a stand-alone document or should it be provided in the Business Proposal.
Answer: This form should be provided as part of the Business Proposal. It is not a stand-alone document.
2. Question: On page 98, RFP Section L.2.a.16, please confirm that bidders are to submit a total of four (4) past performance contracts that have been awarded are ongoing, or have been completed within the last five years.
Answer: Offerors are to submit at least four (4) past performance contracts in their Business Proposal that have either been awarded, are currently being performed or have been completed within the last five years that are similar in nature to the workscope of this RFP. This RFP section, L.2.a.16, is modified in this amendment to reflect this change.
3. Question: In RFP Section L.2.b.2, entitled, “Other Considerations,” please confirm that this requirement can be provided as an appendix to the 30-page technical proposal.
Answer: No, this should be included in your technical proposal.

4. Question: Page 112, RFP Section L.2.b.4, Human Subjects – please confirm the Human Subject section is excluded from the 30-page technical proposal page limit and should be provided as one of the appendices to the technical proposal. If this is excluded from the 30-page page limit, are there any page restrictions on this section?
Answer: The Human Subjects area is a SEPARATE FILE of the Technical Proposal. It is excluded from the 30-page limit of the technical proposal and does not have a page limit.
5. Question: On page 130, RFP Section L.2.b.5, “Enhancing Reproducibility through Rigor and Transparency,” please confirm that this requirement can be provided as an appendix and is excluded from the 30-page technical proposal.
Answer: This is not excluded from the technical proposal page limit and cannot be an appendix.
6. Question: On page 131, RFP Section L.2.B.6, Obtaining and Disseminating Biomedical Research Resources/Data Sharing Plan, please confirm that this requirement can be provided as an appendix and is excluded from the 30-page technical proposal.
Answer: The Data Sharing Plan in summary form should be in the technical proposal and is not excluded from the 30-page limit. An expanded Data Sharing Plan can be an Appendix and is excluded from the 30-page IDIQ technical proposal page limit.
7. Question: Page 132, RFP Section L.2.B.7, Section 508 accessibility standards for HHS Web Site Content and Communications Materials/Product Assessment Template, please confirm this requirement can be provided as an appendix and is excluded from the 30-page technical proposal.
Answer: This requirement can be provided as an appendix and is exempt from the 30-page technical proposal.
8. Question: Page 2, Attachment 1, Section II.B. Please clarify exactly what is to be included in the 30-page limit.
Answer: Please follow the Technical Proposal Instructions on page 110, Section L. 2.b. Also, please follow the ADDITIONAL PROPOSAL INFORMATION AND INSTRUCTIONS, Attachment 5, regarding the technical proposal. There are also 3 other technical proposals, one for Task 1, one for Task 2 and one for Task 3. Each of these have a 20-page limit each.
9. Question: Are offerors permitted to provide an Executive Summary within their response. If so, will this be excluded from the page limitations set forth in the RFP?
Answer: Offerors are permitted to provide an Executive Summary, but it will not be excluded from the technical proposal page limits.
10. Question: Are the locations/clinical that are currently involved static and they would just change to working with the new vendor, (if that were to change).

Answer: The existing network of clinical sites is expected to continue without significant change into the near future. Sites may be added or removed as needed and may be recompeted in the future. Sites are selected by the IMPAACT Network and the Contractor, with input from NICHD.
