Responses to Questions from Potential Offerors:

Question 1:

Could you please clarify whether we are required to submit separate proposals for the RFP and the Task Order? If separate submissions are needed, could you also let us know if all required documents, including the two volumes and the Excel sheet, need to be submitted for both?

Response to Question 1:

The Sample Task Order will be part of the Technical Proposal but will have a separate page limit of 15 pages, with the same formatting and restrictions of the IDIQ Technical Proposal page limit stated in Attachment 1 - Packaging and Delivery of Proposal, formatting and page limitations.

Question 2:

When preparing our responses for the solicitation, should we adhere to the instructions provided in Section L of the RFP, or should we follow the instructions outlined in Attachment 5 (Additional Technical Proposal Instructions, Format for Technical Proposal, and Table of Contents) and Attachment [1] (Packaging and Delivery of Proposals)?

Response to Question 2:

Refer to Attachments 1 and 5 for instructions, formatting, and page limitations that apply.

Question 3:

In Section L of the RFP, which is on page 106, we are asked to describe the experience and qualifications of personnels (Principal Investigator/Project Director). Could you please clarify if we are required to submit resumes for the Principal Investigator /Project Director, or is it sufficient to only describe their experience and qualifications? Also, do we need to submit resumes for the Postgraduate Medical Residents?

Response to Question 3:

See Attachment 5 - Additional technical proposal instructions, Section 4: Contract Management and Personnel, for further instructions regarding Key Personnel and providing CVs for Key Personnel. Additionally see Attachment 1 - Packaging and Delivery of Proposals for CV page limitations.

Question 4:

We noticed a discrepancy regarding the page limit for the technical proposal. Section L of the RFP states that the technical proposal shall not exceed 30 single-sided pages. However, Attachment 1 (Packaging and Delivery of Proposals), - II- B.1 mentions a page limit of 35 pages.

Could you please clarify which page limit we should adhere to for our technical proposal?

Response to Question 4:

Refer to Attachment 1 formatting and page limitations that apply.

AMENDMENT #1, RFP No. 75N93025R00013 Page 3 of 3

Question 5:

We noticed that Section L of the RFP c: Proposal Summary and Data Record (NIH-2043), Page No 100, instructs the offeror to complete the NIH-2043 form. However, no such form or attachment has been provided with the solicitation documents. Additionally, Attachment 6 (Additional Business Proposal Instructions and Uniform Cost Assumptions) Section 1 mentions using the NIH-2043 form as a Proposal Cover Sheet. Could you please clarify if we need to download this form from another source and submit it with our response?

Response to Question 5:

Find attached to this Amendment #1, Section J Attachment # 14_NIH2043 - Proposal Summary and Data Record.

Question 6:

We noticed that Section 4 is not included in Attachment 6 (Additional Business Proposal Instructions and Uniform Cost Assumptions). Could you please confirm if this is an oversight or if there is any specific guidance regarding the missing section?

Response to Question 6:

Confirming that is an oversight and no missing Section 4.

Question 7:

We would like to seek information from the agency regarding Section 7: Other Considerations in Attachment 5, which outlines the Additional Technical Proposal Instructions.

Section 7: Other Consideration section is marked as "N/A." However, in the main RFP document, there is a specified section for "Other Considerations."

Could you please clarify whether the agency expects offerors to respond to this section if applicable, or if it is not necessary to provide a response for Section 7: Other Considerations?

Response to Question 7:

Not necessary to provide a response for Section 7, Other Considerations – N/A.
