This document provides an annual reporting template for federal Surveillance, Epidemiology, and End Results Program contractors. The template includes nine tabs for contractors to report on personnel, budgets, quality improvement activities, data sources and linkages, rapid case ascertainment studies, grants, meetings and trainings, partnerships, and publications. Contractors must provide staffing information including names, titles, roles, and efforts. They must report funding sources and amounts for the current, previous, and upcoming years. Quality improvement activities include audits, tools, trainings, and initiatives led by the National Cancer Institute. Data sources and linkages include estimated records and cases identified. Rapid case ascertainment studies require listing of enrolled patients. Grant information includes funding amounts. Meetings, trainings, and partnerships conducted must also be documented.

Instructions Instructions for Excel Annual Task Order Progress Report
| 1 Registry Personnel |
| In tab '1 Personnel,' provide a list of all registry staff, including those funded under the SEER contract and those who support registry operations but are not funded by SEER. Also include subcontractors on the list. This list should be represnetative of your personnel as of 12/31 of the reporting year. Include: |
| Column A: Name of personnel |
| Column B: Title of personnel |
| Column C: Degrees/Credentials |
| Column D: The role of personnel |
| Column E: Identify who are key personnel as defined by the contract (Y/N) |
| Column F: E-mail address of key personnel |
| Column G: Identify if personnel is a subcontractor (Y/N) |
| Column H: If personnel are funded by SEER contract, provide percent of effort for that individual |
| Column I: Identify percent level of effort of personnel who support registry operations that are not funded under the SEER contract |
| 2 Budget |
| Complete tab '2 Budget,' to provide information and best estimates on your funding sources that were awarded other than the SEER Program to support registry operations. These amounts should be direct dollars and not include indirect costs. Do not include SEER funding or grant funding in this tab (Grant Funding will be included on Tab 7). Include: |
| Column A: Funding Source |
| Column B: Total $ amount awarded for 2018 (calendar year of the report) |
| Column C: Percent of total funding for 2018 (calendar year of the report) |
| Column D: Total $ awardedamount for 2017 (previous calendar year) |
| Column E: Percent of total funding for 2017 (previous calendar year) |
| Column F: Difference between 2018 & 2017 by funding source |
| Column G: Expected funding award for 2019 (upcoming calendar year) |
| Column H: Comments |
| Examples include: state funding, CDC, cancer center funding, state funding |
| 3 Quality Improvement |
| Complete tab '3 Quality Improvement,' to provide information of registry involvement in data quality efforts, regardless of the funding source. The template includes: |
| Column A: List of current quality activities. The initial list is provided by NCI, please add additional registry initiated audits, quality studies to the bottom of the list. |
| Column B: Registry participation (Y/N) |
| Column C: Registry definition of the Quality Activity (fill out if different from SEER Program expectations) (For Training and knowledge assessment section, include whether the registry personnel were the trainer(s) or recipients of the training) |
| Column D: Key contact for the specific quality effort (one person) |
| Column E: Other registry personnel |
| Column F: Describe the level of Involvement by personnel for this quality activity |
| Column G: Specific tasks in progress by the registry |
| Column H: Specific tasks completed by the registry |
| Column I: Comment/Feedback. Provide any input on how useful or not useful the data quality effort is/was or any recommendations for improvement. |
| Add any registry initiated data quality activities that are not included in the excel tab. |
| 4 Linkage Data Sources and Counts |
| Use Excel tab '4 Linkage Data Sources & Counts,' to list all data sources that inform or populate SEER data. This includes any linkages that your registry has done in the past or is currently doing. Include any linkages that are done at the central registry. The template requires: |
| Column A: Name of data source or linkage |
| Column B: Type of data (i.e. pathology, radiology, EMR, genomic, radiation, pharmacy, etc.) |
| Column C: Purpose of Data Source (i.e. incidence/case finding, follow up, detailed treatment, capture biomarkers, increaswe completeness, determine survival, etc) |
| Column D: Frequency of Linkage (i.e. real time, annual, quarterly) |
| Columns E: Years of data that has linked with registry |
| Column F: Estimated number of source records this year |
| Column G: Estimated number of cases (CTCs) this year |
| Examples include: DMV, Social Security Administration, National Death Index, CVS, Walgreens, Unlimited Systems, Varian, Elekta, e-pathology reports, state all payer claims, LexusNexis/Accurint. |
| Please include a comprehensive list of all data sources, that may or may not be captured through SEER*DMS. IMS will provide technical solutions and support. If the registry does not have an exact count for the data source, provide the best estimate documents and cases by source. |
| 5 Rapid Case Ascertainment (RCA) & Patient Contact Studies |
| Using tab '5 RCA & Patient Contact Studies,' list clinical trials and research studies using real time case ascertainment. Rapid case ascertainment should include identifying eligible cases for research purposes, regardless of the number of days. Include: |
| Column A: Name of study |
| Column B: Type of study (i.e. cohort, clinical trials) |
| Column C: Site/Histology of Study (if applicable) |
| Column D: Start Date of Study |
| Column E: End Date of Study |
| Column F: Number of patients evaluated this year (everyone possibly reviewed for eligibility and enrollment) |
| Column G: Number of patients enrolled this year (patients actually enrolled in study) |
| Column H: Funding source for the study (i.e. NCI, NIH, Cancer Center, specific non-profit) |
| Include any grant information from RCA & Patient Contact Studies in " 6 Grants." |
| 6 Grants |
| In tab '6 Grants', provide a list of grants that included the SEER PI or other SEER personnel or grants at your institution that utilized SEER data. Include: |
| Column A: Full NIH Grant Number (if applicable) |
| Column B: Source of Grant (i.e. NIH, CDC, name of foundation, other federal sources) |
| Column C: Contact PI |
| Column D: Project Title |
| Column E: How was SEER data used for the project: |
* Main analytic data source

* Used to identify patients or study samples

| Column F: Total Funding Amount for Grant for Calendar Year |
| Examples of grants can include: NIH, CDC grants, foundation grants such as American Cancer Society, PanCan, etc. and other federally funded grants. Do not included unfunded research activities in this tab. |
| 7 Key Meetings and Trainings |
| List presentations or posters completed by SEER funded personnel in excel tab '7 Key Meetings and Trainings.' Include: |
| Column A: SEER authors/presenters |
| Column B: Title of presentation/poster |
| Column C: Name of Conference |
| Column D: Date(s) |
| Meetings that could be included but not limited are: NAACCR, NCRA, AACR, ASCO, CI4CC, AACI, ASTRO. |
| List trainings that CTR registry personnel led and facilitated in the second part of excel tab '7 Key Meetings and Trainings.' Include: |
| Column A: The names of SEER funded Trainers |

| 8 Partnerships |
| List partnerships and collaborations and the nature and purpose of the partnership in excel tab '8 Partnerships.' These partnerships can include government agencies, industry, or other collaborators not mentioned in previous sections. |

| 9 Publications |
| The Publications tab will only be filled out if the registry does not maintain a bibliography list using reference management software, such as EndNote or RefMan. |
| Review the SEER Publications Database for publications that include at least one SEER registry personnel (either directly supported or through cost share) and/or utilize SEER data. Identify any missing peer-reviewed publications, white papers, or other publications by the registry (i.e. registry reports that are utilized by the State Department of Health). The publication database: https://seer.cancer.gov/publication-database . For any log in requests for additional SEER personnel, contact Jessica Boten, jessica.boten@nih.gov |
