Instructions
Block 1- Select the appropriate schedule type:
Schedule I-A: Contractor Acquired Government Property - Equipment has been determined to be needed and contractor is authorized to Purchase the specific item(s) upon award of contract.
Schedule I-B: Contractor Acquired Government Property - Need for the equipment is recognized and it is anticipated to occur later during contract performance. COA is needed for contractor to purchase equipment.
Schedule II-A: Property Acquired under predecessor Contract No. _________________ Will be furnished from a predecessor contract; inventory reconciliation and transfer documentation will need to be completed.
Schedule II-B: Government furnished Property - Need for specific items of equipment has been recognized and is anticipated to occur later during contract performance. Equipment may be furnished from the Government inventory or purchased by the Government.
Block 2- Prime Contract Number: Enter Contract Number.
If the property applies solely to one contract modification indicate the modification number as well.
Block 3- Prime Point of Contact and Address: Enter Contractor’s point of contact and address Block 4 - Number of Items: Indicate how many items are listed in Block number 11 Block 5- Total Acquisition total: Enter the value of the property Block 6- Date of Acquisition Purchased: Enter the purchase date of property Block 7- Location of Property: Enter address of where the property will be held/used.
Block 8- Points of Contact for Property: Enter point of contract that will be responsible for the property.
Block 9- Item No. : Self explanatory Block 10- Item Description: Describe each item in sufficient detail (i.e item, size, brand, etc.)
Block 11- Property Classification: Use one of the following classifications for each line item:
EQ – Equipment M – Material STE – Special Test Equipment ST – Special Tooling
Block 12- Serial Number: distinguished number to indicate the property.
Block 13- Quantity: list the quantity of items.
Block 14 - Cost (Unit/Total): Indicate the cost of each unit and total.
