RFP Template Revised 11-9-2020
| CCRT Deployment Scenario Staffing Roster. Indicate names of qualified individuals in spaces below. | ||||
| AMENDMENT 0001 – REQUEST FOR PROPOSAL (RFP) 75H70421R00002 changes to Appendix L include: Delete Zuni IHS Hospital, Gallup Indian Medical Center, Red Lake Hospital and Rosebud Hospital from scenarios. A CCRT team is scheduled to deploy to these locations under Purchase Order 75H70421P00002. If offeror is awarded an IDIQ contract, there is a strong likelihood that IHS will issue a request for task order proposal, for one or more of the locations on Attachment A, which could include the locations identified on Appendix L, but actual deployment needs are subject to change. Appendix L is for purposes of responding to REQUEST FOR PROPOSAL (RFP) 75H70421R00002 and does not commit the government. | ||||
