About this file
This is an Amendment (A00001) to a Request for Quote (RFQ) issued by the Indian Health Service (IHS) for decommissioning four wells located in the White Swan area. The amendment extends the quote due date to August 14, 2025 at 2:00pm PT and provides detailed responses to six contractor questions regarding well decommissioning procedures. Key clarifications include: confirmation that three wells (White Swan Well #3, South White Swan Well #1, and South White Swan Well #4) have drop pipe and pumps installed, guidance on potential well decommissioning methods requiring approval from the Washington State Department of Ecology and Yakama Nation Water Code Administration, and instructions for site access and fencing removal. Contractors must coordinate well site access with IHS and tribal representatives and must follow all specified codes and standards when performing the decommissioning work.

Amendment A00001 – SF 30, Block 14 (Cont’d)
1. The following questions received in response to this solicitation have been answered:
NO. QUESTION ANSWER
1 If we had permission from the Washington State
Department of Ecology would IHS allow for the wells to be decommissioned by using bentonite "hole plug" and cutting off 2' from the surface instead of perforating from the bottom?
This method must be approved by the State of Washington and the Yakama Nation Water Code Administration (POC
Andrew Smith, andrew_smith@yakama.com, 509-865-5121 ext. 6126). All State and Tribal permits and requirements must be met.
Note, IHS has attempted to coordinate a response to this question with the State of Washington and the Tribe, but no official approval has been received from either party and IHS is unable to authorize deviation from the codes and standards currently within the solicitation.
2 Can you please provide the well log for the nearest well to well #4?
The best information we have on each well is provided in the solicitation and attachments. The Well #4 info was provided from a site inspection so is rather scant on details.
3 Do you know which wells still have pumps in them? The SOW (solicitation Section C) provides information on each well including whether there is drop pipe and pump in place. Three of the four wells to be decommissioned (White
Swan Well #3, South White Swan Well #1, and South White
Swan Well #4) all have drop pipe and pump still installed.
4 Reference Well #4 Photos: In the first picture is looks like the well is inside a building. Is this building being torn down, or will we need to decommission it by applying for special standards within the building?
The building is to remain intact and the well decommissioned as pictured according to the applicable codes and standards provided within the solicitation.
5 Reference Well #1 Photos: In the second picture, are we able to take down some of the fencing to give us access for our rig and tooling swing?
Fencing may be temporarily removed or displaced if coordinated in advance with the IHS COR and Tribe. Any fencing disturbed must be replaced or reinstalled upon completion of the decommissioning.
6 Can we visit the well sites? If the intent is to see the well locations and observe from the outside, you may do so at any time without coordination with
IHS. If access to the wellheads is desired, we will need to coordinate timing with both IHS and the Tribe by contacting
Toby Hayden at toby.hayden@ihs.gov and Jason Davis at
Jason.Davis@ihs.gov.
2. The quote due date is extended to August 14, 2025 at 2:00pm PT.
3. All other terms and conditions remain unchanged.
Offerors are reminded to acknowledge all amendments in block 19 of the SF1442 when submitting a quote in accordance with the RFQ requirements.
