| Service Center Matrix - Template Instructions |
| Attachment J.15 is utilized to support proposed level of effort in a service center. |
This template is to be filled out by:
(1) Offerors and
(2) proposed subcontractors that would be performing more than 10% of the level of effort under any functional area within Sections E.1 through E.6 of the Statement of Work (SOW) where multiple MAC or MAC/legacy contracts are involved.
The Offeror shall complete Attachment J-xx to provide its service center personnel listing, with names and positions pertinent to any existing Legacy Title XVIII contracts and awarded MAC contract(s), in addition to the PDAC work. The Offeror shall provide this personnel listing for all contracts for the time period of CLIN 0002 of the PDAC Base year.

| Company Name | Fill in your company name on each worksheet in the space provided next to "COMPANY NAME". If you are a subcontractor, also fill out Row 2. |

| Proposed Subcontractor for: | If you are being proposed as a subcontractor for the PDAC contract, please fill in the Offeror you are being proposed as the subcontractor for in Row 2. EXAMPLE: Company Prime is proposing using Company Sub4U as a subcontractor for the JK contract. When Company Sub4U fills out this form, Row 1 says "Sub4U" and Row 2 would indicate that they are a subcontractor for "Company Prime". |

| Service Center Name | Identify each service center proposed on this contract. |
| Column A, | |
| Proposed Position | Identify the labor category, which should crosswalk to the position descriptions provided under Tab C.2.2. |
| Column B, | |
| Employee Name | Provide the name of the FTE proposed for the labor category, or identify the employee as "new hire." Only list one name in each row of this matrix. |
| Column C, | |
| Experienced / Inexperienced | Identify each FTE as “experienced” or “inexperienced.” The “experienced” label should be used for existing staff or new hires that have experience working in the Medicare Fee-For-Service Program, or equivalent work experience. NOTE: this data should be reflective of your proposed staffing approach for the PDAC contract. |
| Columns D through J | |
| SOW Section/FTE Allocation | Provide the service center FTE allocation and PDAC SOW functions(s). The PDAC SOW function(s) should be numerically coded to correspond to the PDAC SOW. The FTE allocation should be represented as either 1.00 or less than 1.00. The totals for each person cannot exceed 1.00. |
| Column K, | |
| Total | Total the FTE allocations for each named FTE across all contracts. The totals for each person cannot exceed 1.00. |
| Provide the total FTE allocations by contract using the bottom row of the matrix. Further, identify the total service center staff proposed for the PDAC contract and the allocation percentage of the proposed PDAC staff. |
NOTE: A sample listing is provided in the "Service Center Matrix" sheet to assist the Offeror in completed this Attachment.
