About this file

This document includes a past performance questionnaire and details regarding a federal contract opportunity for providing virtual instructional design and training services. The U.S. Food and Drug Administration is seeking a contractor to develop and deliver meaningful virtual training, transition existing in-person courses to virtual formats, and create new virtual instructor-led training courses. Services required include instructional design, facilitation, course production, and training on effective virtual teaching techniques. The contractor must demonstrate experience facilitating engaging virtual learning environments, selecting appropriate methods, and dealing with technical or participant issues. The past performance questionnaire requests references to evaluate the offeror's relevant previous work. Responses are due by email to the FDA contacts provided by the deadline in order to be considered for this contract award.

Attachment 2: Past Performance Questionnaire

Dear Sir or Madam:

The U.S. Food and Drug Administration (FDA), has a requirement to procure services for providing skilled instructional design services necessary to facilitate and deliver meaningful training virtually and to adjust OTED’s current training plan and model to successfully achieve and maintain the transition of ILT to vILT and create new vILT courses.

FDA is conducting a performance confidence assessment in anticipation of a possible contract award. An offeror interested in proposing on this work has identified you as a Point of Contact (POC) on a past or present contract which the Offeror deems relevant to this effort. FDA is requesting your assistance in completing the attached Past Performance Assessment Questionnaire so that we may evaluate the offeror under the Factor of past performance. The Questionnaire has been developed for ease of electronic completion. Please, provide your comments regarding the overall assessment of the Offeror’s performance on the contract identified and any additional information that your organization deems relevant to the FDA evaluation team. It is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators.

Please complete and submit the Questionnaire via e-mail to Contract Specialist at Pamela.Lee@fda.hhs.gov. and Contracting Officer Madeline.Bryant@fda.hhs.gov if you have any questions relative to the enclosed Questionnaire, please contact me.

Thank you in advance for your assistance
