SECTION SF 30 BLOCK 14 CONTINUATION PAGE

SUMMARY OF CHANGES

1. The Centers for Disease Control and Prevention (CDC) is revising the requirement to award a single Blanket Purchase Agreement (BPA) for dry ice and liquid carbon dioxide (CO2), but reserves the right to award a single BPA for dry ice and a single BPA for liquid carbon dioxide (CO2) from this solicitation. Contractors may propose on the dry ice requirement or liquid carbon dioxide (CO2) requirement or both. Award, whether for the total requirement or individual requirements will be based on lowest price, technically acceptable procedures.

2. This Amendment 0004 is the first SF30 amendment, and the amendments annotated on SAM.gov are changes to the set-aside and due date only, so the solicitation was not updated, only the SAM.gov posting. This amendment rectifies and updates the full solicitation.

3. The due date for questions to be answered using the Q&A spreadsheet is extended to Thursday, September 19, 2024, at 12:00 PM EDT. Please review the previous Q&A spreadsheet to determine if the question has already been answered.

4. The due date for quote submittal is extended to Thursday, October 10, 2024, at 12:00 PM EDT.

5. The Offeror will be expected to include all delivery fees in their quote and annotate that delivery fees are included or not included.

6. It is estimated the delivery of dry ice at the Lawrenceville campus to be once a week (usually Mondays), 52 deliveries a year. For the Roybal campus, delivery of dry ice is estimated at 2 deliveries per week (usually Tuesday and Thursday) which amounts to 104 deliveries a year. Bulk CO2 is delivered based on usage and can be obtained from the provided telemetry. Usage across the previous years has shown that it is pretty much every three weeks, so 17 per year. The smaller tanks are filled at the same time as the primary, unless there is an issue, and a tank is isolated - since the piping is interconnected. CO2 cylinders, when needed, are also based on usage but would probably need to be refilled or replaced 2-3 a quarter standardly, 12 a year.

7. CDC will accommodate a site visit for 1 to 2 persons per company if requested. No site visit will be allowed after Friday, October 4, 2024. Site visits will be for the liquid CO2 delivery site. Pictures of the site can be provided to offerors who cannot make a site visit. Requests for a site visit must be received by September 19th, 2024, at 12:00pm EDT. Site visits will be scheduled as group site visits. Site visit requests should list preferred days and times. FAR 52.237-1 (Site Visit) and FAR 52.237-2 (Protection of Government Buildings, Equipment, and Vegetation) are incorporated into this amendment.

Not-to-Exceed Summary

Year 1 Year 2 Year 3 Year 4 Year 5

Not-to-Exceed Ceiling: ______________________

Estimated Number of Deliveries:

Dry Ice Roybal: 104 per year (2 deliveries per week) Dry Ice Lawrenceville: 52 per year (Once a week) Roybal CO2: 17 per year (Once every 3 weeks) CO2 Cylinders: Roughly 12 a year.

B3. Scope of Works The deliverables are the dry ice and CO2 needed for CDC laboratory operations and associated activities at the CDC Lawrenceville and Roybal campuses. This may be in the form of dry ice or solid CO2, bulk liquid CO2 tank refills or leased cylinders of CO2. Dry ice is the frozen form of carbon dioxide. Instead of turning from a gas, into a liquid, and then into a solid, carbon dioxide goes straight to a solid form. As a result, when it melts it also turns right into gas. Dry ice is extremely cold and has a surface temperature of -109.3 degrees. Dry ice is so popular because it’s easy to freeze and relatively easy to handle, as long as proper techniques are employed. It eliminates the need for wet ice and the spills that can accompany this method. The use of gaseous CO2 for laboratory incubators means gathering the flow requirement in normal operation for each incubator supplied from the same source, recognizing that flow is non-continuous. Rather, flow is dependent on the equipment’s duty cycle, being affected by how often the chamber is opened to remove or replace samples. This is expressed in liters per minute (lpm) of CO2 flow, at six lpm for each incubator, doubling the rate for dual stack incubators. The CO2 will maintain the required concentration inside the chamber, typically 5 percent, with a balance of air or N2 to limit the oxygen content.

OutBreak/Active operations use a 30 percent duty cycle while normal operations use 10 percent, comparing duty cycles against total daily and weekly CO2 demand per incubator. Totaling weekly demand per incubator will provide the net weekly CO2 requirement for the system, as the CO2 availability should be sufficient to supply one week’s operation. The gaseous CO2 purity shall meet the minimum standard of anaerobic grade with a 99.95% rating.

B.4 Extent of Obligation The Government is obligated only to the extent of authorized purchases actually made under the BPA through GPC orders. The Government’s maximum liability for this BPA is $0.00. The Government estimates, but does not guarantee, the BPA may total up to $4,800,000.00. It is the responsibility of the individual ordering activities to ensure adequate funds are available for CDC GPC purchases. This document does not obligate any funds.

B.5 Period of Performance The Period of Performance of this BPA is sixty (60) months from the effective date in Block 3 on page 1.

The delivery date for each order shall be determined under each individual order through the mutual agreement of the parties involved. The actual delivery of chemicals may extend beyond the BPA period for a maximum of thirty

(30) days.

Any order issued during the effective period of performance of this BPA and not completed within that period shall be completed by the Contractor within the time specified in the individual order. The BPA shall govern the Contractor’s and Government’s rights and obligations with respect to that order to the same extent as if the order were completed during the BPA’s effective period; provided, that the Contractor shall not be required to make any deliveries under this BPA after thirty (30) days following the end of the period of performance of the BPA.

B.6 Ordering Orders will be placed through the issuance of individually placed CDC GPC purchases. Telephone or other verbal orders are not authorized under this BPA.

Issuance of Orders All orders are subject to the terms and conditions of the basic BPA. In the event of a conflict between an order and this BPA, the BPA shall control.

Requirements issued under this section shall be referred to as “order requests” until approval of the final order issued by the Contracting Officer, or a CDC GPC holder identified in Section B.10.

The following information details the sequence by which CDC GPC orders will be issued:

As supplies are identified to be placed under the BPA, the Center/Institute/Office (CIO) shall notify the Contract Officer Representative (COR) and submit an order request setting forth the specific items to be purchased. The specific items will vary from order to order, but shall consist of one or more of the items listed under Schedule B. The COR shall review and approve the order request. One of the individuals authorized in B.10 will then place a CDC GPC order against this BPA.

Established cost ceilings shall not be exceeded without the prior written authorization of the Contracting Officer.

B.7 Pricing Pricing to the Government shall be as low, or lower, than that charged to the suppliers most favored customers for comparable quantities under similar conditions, in addition to any discount for prompt payment.

B.8 Delivery Terms Delivery Locations Deliveries shall be made F.O.B. Destination. Deliveries will be made to one or more of the following Government facilities:

CDC – Roybal Campus (Atlanta, GA) CDC – Lawrenceville Campus (Lawrenceville, GA)

1. Name of supplier

2. BPA number

3. Date of purchase

4. Purchase number

5. Itemized list of chemicals furnished

6. Quantity, unit price, and extension of each item, less applicable discounts (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show this information)

7. Date of delivery or shipment

B.9 Purchase Limitation The total aggregate amount of this BPA is $4,800,000.00. This amount is estimated at the time of issuance of this BPA and does not represent the guarantee that any purchases will be made. Neither does this estimate represent a ceiling for the amount of purchases that are made and may be exceeded if such action is determined to be in compliance with Federal law by the Agency ordering official.

The dollar limitation for each individual CDC GPC purchase under this BPA is $10,000.00. The dollar limitation may not be exceeded except by bilateral modification to accommodate additional needs of the agency.

By signing this BPA, the Offeror guarantees to provide an uninterrupted source of supply of the originally awarded item(s) from which to satisfy the Government’s requirement for the Period of Performance of the awarded BPA.

B.10 Individuals Authorized to Purchase under the BPA The individuals authorized to make CDC GPC purchases are as follows.

The identified individuals are authorized to make CDC GPC purchases under this BPA up to $10,000.00 per transaction. Additional Card holders may be added after the award date. This will be done by a BPA modification.

B.11 BPA Modifications This BPA may be modified by mutual agreement of the Government and the Contractor. Each modification shall be in writing, issued by the Centers for Disease Control and Prevention (CDC), and signed by the Government and the Contractor. Modifications shall become effective on the date signed by the Government. Certain modifications may be issued by the Contracting Officer on a unilateral basis in accordance with FAR 43.103(b).
