ASSISTANT SECRETARY FOR PREPAREDNESS AND RESPONSE (ASPR)

STATEMENT OF WORK (SOW)

ASPR

EMERGENCY RESPONSE SUPPORT SERVICES (ERSS)

TO

SUPPORT US GOVERNMENT TRANSPORTED INDIVIDUALS

IN THE

CONTINENTAL UNITED STATES, OUTSIDE THE CONTINENTAL UNITED STATES, U.S. TERRITORIES (CONUS and OCONUS) AND OTHER HOST NATIONS 06/12/2019

1.0 INTRODUCTION

The National Response Framework designates the Department of Health and Human Services (HHS) as the lead agency for Emergency Support Function #8 (ESF-8), Public Health and Medical Services and provides the mechanism for coordinated Federal assistance to supplement State, tribal, local and host nation resources in response to a public health and medical disaster or incident. The Office of the Assistant Secretary for Preparedness and Response (ASPR), under the Office of the Secretary has been designated as the lead for an ESF-8 response to a public health and medical incident or disaster.

When the President declares a major disaster declaration in the United States for any of its states, tribal entities or territories, the Federal Government to include ESF-8 can be activated to provide supplemental public health and medical assistance.

This assistance could be in whole or in part to assist local health authorities assist displaced or local populations, assess and monitor ongoing health and human services needs of affected populations and support “at-risk” individuals and populations. This may include support of high acuity patient care operations that could exceed 1,000 patients for each site and could include multiple concurrent sites of operation directed by HHS and include non-medical attendants and associated family members of patients. The Contractor shall respond within 2 hours to the Contracting Officer’s (CO) phone call/e-mail notifying them of an emergency. This is the CO notifying the contractor that the SOW is forthcoming. The Contractor shall respond with a proposal within 48 to 72 hours of the CO’s Request for Proposal (RFP) in a response to an emergency. Each RFP will have the defined timeframe for a response. Mobilizing resources required at the task order level shall begin within 24 hours to 1 week after receipt of a task order. CONUS mobilization from 24 hours to 4 days and OCONUS 24 hours to 1 week, this will be determined per task order/per response. Surge (this is a response to an existing task order where a modification is needed) response proposals shall be provided within 24 hours to 72 hours and will be determined per task order/per response. Mobilizing resources required at the task order level for surge response CONUS shall begin with 24-48 hours after receipt of a modification and OCONUS within 48-72 hours after receipt of a modification.

This requirement is a comprehensive support mechanism and the Contractor may be required to provide all of the support, but only specific parts of this requirement will be needed on each task order level. The Contractor may be required to set up a field site.

*At the discretion of the Government the Contractor could be required to pre-stage materials. These directions would be formalized as a Task Order through the CO prior to being implemented.

2.0 PURPOSE AND OBJECTIVES

The purpose of this Statement of Work (SOW) is to obtain a comprehensive solution to provide services for the areas listed above (Chart 1) in support of medically displaced populations and/or inpatient clinical operational sites CONUS and OCONUS to include US territories or Other Host Nations (foreign countries, i.e. response events like Haiti, Japan, Iran, Liberia) at one or multiple locations separately or simultaneously, based on mission requirements.

3.0 PERIOD OF PERFORMANCE AND CONTRACT TYPE

This will be a firm-fixed price (for labor categories only) base single award Indefinite Delivery Indefinite Quantity (IDIQ) contract, with time and material or labor hour task orders, and Cost Reimbursement for Other Direct Costs (ODC) Contract Line Item Numbers (CLINS) for materials, shipping and travel. The period of performance will be five years, comprised of one 12-month base period beginning on the date of contract award, and four (4), 12 month option periods exercisable at the Government’s sole discretion, in accordance with Federal Acquisition Regulation (FAR) 52.217-9. The Government reserves the right to extend the contract for up to an additional six (6) months in accordance with FAR 52.217-8. Individual task order length will be defined in the developed SOW for each task order.

4.0 PROGRAM MANAGEMENT AND PROJECT PLANNING

The Contractor shall furnish all necessary personnel, materials, services, and do all things necessary to execute the SOW. In performance of this contract, the Contractor shall complete the objectives for each component.

The Contractor shall provide services in geographic areas directed by the CO per each task order. The Government will evaluate Contractor personnel proposed upon receipt of proposals and at the task order level following award. The Contractor is expected to perform this mission support activity with economy, lean manning and materials with unique approaches. The contractor shall bring issues to the Contracting Officer Representative (COR) and CO. Further, the Contractor shall employ a method of conservation of supplies and shall receive authorization from the CO before purchasing materials or supplies and before shipping any equipment. The Government may or may not be providing equipment/supplies, which will be addressed at the task order level. The quantities, types, and locations of the services either provided by the Government or supplied by the Contractor may change (increase/decrease) within the Period Of Performance (POP) and these changes will be directed by the CO in the form of a modification and funded accordingly. Any equipment bought with federal funds will be the property of the federal government and will be tagged with HHS tags.

The Contractor shall provide life support to their own forward deployed personnel and subcontractors. This includes, but is not limited to, office facilities, office supplies, Information Technology (IT) equipment, billeting, communications support, meals, and transportation, which will be addressed in each task order and invoiced under the ODC CLIN.

The Contractor shall appoint a Program Manager (PM) who will be the primary point of contact (and a Key Personnel) for the CO and the COR. The PM shall have resources and authority to endure efficient and timely program execution and shall be the Contractor’s focal point for all required tasks. The Contractor’s PM shall be prepared at all times to present and discuss the status of contract activities, requirements and issues and possible solutions. The name of the PM and the name of any alternate who shall act for the Contractor when the Program Manager is absent shall be submitted in writing directly after award. During any absence of the PM, only one (1) alternate shall serve as the Government point of contact. The PM and any designated alternates(s) shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract and be available 24/7/365. The Program Manager shall keep the CO and COR informed of any performance issues, cost or financial concerns and potential problems that if unresolved, will adversely affect the Contractor’s performance, schedule, costs and the mission. The PM shall take all appropriate measures to mitigate adverse impacts to the contract and subsequently to the mission. The contractor shall provide an action plan with each task order.

5.0 HOURS OF OPERATION

The Contractor shall perform all services under each task order 24 hours a day, 7 days a week including official holidays, unless otherwise stated in the SOW. If any services are identified for less than 24 hours a day, the Contractor shall provide on-call personnel for 24 hour operations in case of emergency.

6.0 REPORTS

The Contractor shall provide performance and status reports to the Government as the frequency and type determined by the task order.

7.0 PERSONNEL

The Contractor shall provide the necessary personnel with appropriate skills, certificates and licensing required to perform services identified in the Contract/SOW.

a. The Contractor will assure all labor categories requiring licenses and renewals for credentials are maintain consistent with the Federal Laws/regulations specific to each discipline. Labor category pricing will be fully burdened rates. The Contractor may use a licensed employee holding required licenses and certifications from any US State or Territory (being they do not need licensure in the State/Territory where the work will be occurring). It is incumbent on the Contractor to verify the licenses and certifications are current. This applies to any worker on the contract.

b. The Contractor shall be responsible for ensuring all personnel comply with the standards of conduct, and all terms/conditions set forth in the SOW and the Contract. The Contractor shall provide the necessary supervision for personnel required to perform the work under this contract.

c. The Contractor shall use its best efforts to hire Local National (LN) personnel and Subcontractors when appropriate, for cost saving effectiveness. Oversight and compliance with all terms of the contract remain the responsibility of the Contractor.

8.0 KEY PERSONNEL

The Contractor shall designate a full-time Program Manager (PM) and alternate(s). The Contractor PM (or alternates) shall be onsite and responsible for overall contract performance, and shall have full authority to act for the Contractor on all matters related to contract operations under their purview. The PM (or alternate) shall be available by email and phone at all times during an event.

The Contractor shall electronically provide to the CO and COR, prior to contract start date, name and phone number of the PM along with the names and phone numbers of any alternate, if applicable. The Contractor shall electronically provide the CO and COR with updates to this list, if the information changes, no later than (NLT) ten (10) calendar days prior to the change.

The PM (or alternates) shall meet, as necessary, with Government personnel designated by the COR or CO to discuss immediate administrative and technical issues and problem areas.

Each Task Order may need its own PM, to oversee each specific task order, this determination will be made with each task order.

9.0 PROPERTY CONTROL

The Contractor shall submit a Property Control Plan per task order for property federally funded or provided. All property shall be inventoried in accordance with (IAW) the Contractor's Government approved Property Control Plan and Federal Acquisition Regulation (FAR) Part 45.

10.0 SAFETY AND HEALTH

The Contractor shall submit to the COR a Safety and Health Plan for tasks identified in the task order level SOW. This is an Occupational Health requirement for Contractor Personnel.

11.0 ENVIRONMENTAL PROTECTION

The Contractor shall comply with all environmental protection requirements as outlined in the Contract.

12.0 PROJECT SCHEDULE

The Contractor shall submit to the COR and CO Project Schedules In Accordance With (IAW) each task order. The Contractor shall provide sufficient detail to demonstrate that all sections of the work are appropriately resourced, thoroughly planned and proceeding according to requirements, to include milestones to demonstrate successful completion of projects. Schedule updates shall be made frequently, or as designated by the COR. Changes to scheduled baseline milestones shall be documented by the Contractor and formally authorized by the CO/COR.

13.0 SECURITY CLEARANCES AND OPERATIONAL SECURITY (OPSEC)

Top Secret, Secret, etc., security clearances are not required. The contractor will conduct a basic background check (e.g. criminal history, credit check, finger prints, public trust, etc) on all prospective candidates for the government’s consideration for acceptance in conducting task orders.

The Contractor shall coordinate with the COR and CO to determine when changes occur and whether these changes have an impact on the Contractor’s performance.

14.0 CONTRACTOR SECURITY REQUIREMENTS

The Contractor shall comply with vetting/badging policies for all personnel IAW HHS policy.

15.0 MARKINGS

The Contractor shall not place their name, mark, or symbol on any signs, maps, drawings or items without the specific written approval of the CO except to include a reference to Emergency Response Support Services (ERSS) contract and the specific contract number under which this document was developed.

16.0 ESSENTIAL SERVICES

The Contractor shall develop a Mission-Essential Contractor Services Plan for those tasks that have been identified as essential to provide reasonable assurance of continuation during crisis conditions. The Contractor shall ensure compliance with all planning, preparation, and information requirements identified by U.S. Government provided guidance applicable to the specific Area of Operation (AO).

17.0 DELIVERY SCHEDULE

The Contractor shall ensure performance is provided IAW the delivery schedules set forth in the Contract down to the task order level.

18.0 GOVERNMENT INVESTIGATIONS

a. The Contractor shall facilitate external investigations and shall conduct internal investigations. The Contractor shall not restrict or impede the Government’s access to locations where contract services are being performed. This also includes providing unrestricted access to Subcontractor locations in support of this contract.

b. The Contractor shall assist the Government with internal investigations and report of findings for losses and discrepancies of material, inventory, property, or other loss.

19.0 DETAILS FOR AREAS UNDER CHART 1

19.1 INCIDENT MANAGEMENT TEAM

The Contractor shall provide Incident Management Team (IMT) functions and employ an Incident Management structure in accordance with the National Incident Management System (NIMS) while performing IMT functions. The Contractor will only be responsible and have oversight for their own employees and their own sub-contracts. All inherently governmental functions will remain with the Government and will be communicated to the Contractor solely through the COR and CO. All meetings the Contractor attended must be co-attended by the COR or CO. The IMT shall provide all Contractor to Government reporting, logistical, operational, financial and planning support necessary regardless of type, size or complexity of the mission and perform the following functions:

· Report to and coordinate daily with the CO/COR identifying any potential changes in mission scope, issues that arise with provision of services or any critical change or threat to mission personnel well-being for each task order.

· Develop and provide a formal Incident Action Plan weekly which includes forms such as ICS 209 Incident Status Summary, ICS 202 Incident Objectives (24, 48, 72 hours), ICS 205 Communication Plan, ICS 215 Hazard Risk Analyses, and ICS 215 Operational Planning Worksheet, ICS 203 Organization Assignment List, ICS 204 Assignment List, ICS 206 Medical Plan, ICS 208 Safety Message Plan, ICS 214 Activity Logs.

· Provide language access services (interpretors or otherwise) to address the needs of individuals with limited English progiciency, non-English speaking, or other communication disabilities.

· Establish a reporting schedule, timely development and dissemination of the Situation Report and Incident Action Plan weekly or agreed upon per task order. This will be a collaboration between the COR and the Contractor.

· Submit all work products to include, meeting minutes for formal and informal meetings to include conference calls, notes, worksheets, electronic or written in a time frame agreed upon with the COR.

· Upon completion of contract performance, the Contractor shall provide the Government all Privacy Act Information including Health Insurance Portability and Accountability Act (HIPAA) on the evacuees and shall not retain any privacy act information.

19.2 TRANSIT/PARATRANSIT/SHUTTLE/AMBULANCE SERVICES

The Contractor may be required to provide both emergency based and scheduled high aculity emergency ambulance services for US Government Transported patients, NMAs, and associated family members in temporary housing and patients being transported to and from airfields or other transit mechanisms to care facilities. The Contractor shall provide all coordination, dispatch services, vehicles, equipment and staff (to include qualified medical personnel) for ambulance services.

The Contractor shall provide emergency transportation to transport patients, NMAs, and associated family members to urgent care facilities or emergency room for non-life threatening needs within an hour of being notified. Emergency transportation for a life threatening need, to the hospital, would be provided by local EMS providers, unless the disaster area is overwhelmed and EMS is not a viable option.

The Contractor may also be required to provide appointment-based and general transportation (paratransit and shuttle) for US Government Transported patients, NMAs, and associated family members. The Contractor shall furnish all labor, management, travel, supplies, equipment, materials, and vehicles to perform the work described herein, unless otherwise specifically stated. The Contractor shall comply with applicable federal, state, and local laws, regulations, and guidance; and perform all work in accordance with these requirements and any applicable agreements. Transportation scenarios can include, but are not limited to: US Government Transported patients, NMAs, and associated family members requiring medical treatment need appointment-based transportation multiple times a week from their lodging locations to treatment centers; US Government Transported patients, discharged from medical facilities to designated locations; US Government Transported patients, and/or NMAs to follow up medical appointments; transportation to or from the airport; and planned group trips to churches, shopping needs, and other trips approved by the COR.

Transit/Paratransit/Shuttle and/or Ambulance Service Objectives:

· Develop a transportation plan that integrates shuttle and appointment-based transportation services to meet medical, social services, recreation, and personal appointment needs.

· Ensure the vehicles are not more than 10 years old, from the current date of operation.

· Provide a dispatch-managed full service transportation 24 hours a day, 7 days week; with peak service hours being 5:00 am - 9:00 pm. The Contractor shall manage dispatch requirements, transport schedules and requests for transport to ensure timely transportation to appointments.

· Schedule transportation to ensure the prompt pick up times of the US Government Transported patients to maintain on time appointments. US Government Transported patients, will only be allowed a 15 minute tardy window. The driver(s) must wait 15 minutes after pick up time for the US Government Transported patients, (if pick up time is scheduled for 9:00 am the driver must wait until 9:15 am before leaving).

· Provide and respond to critical medical transportation requests (medical appointments, pharmacy) within 1-hour of notification.

· Provide EMS staffed ambulances for transporting medically fragile patients from air heads or other conveyances to points determined by the Government such as care facilities.

· Provide a sufficient number of general purpose and Americans with Disabilities Act (ADA) compliant passenger transport vehicles.

· Maintain a serviceable transportation fleet that meet DOT inspection standard, service ADA loading and unloading equipment, and appropriate safety equipment such as fire extinguishers, breakdown markers. Operators must be properly licensed in accordance with federal and state requirements.

· Provide courteous customer service during operations and driver or and assistant should be able to assist non-ambulatory clients in and out of vehicles, and assist into facilities if other support is not available, if required by the patient.

· Provide the COR a weekly Transportation Services Report detailing number of all trips and the number of on schedule and delayed transportation mission(s). All delayed mission(s) should be explained with a service remediation plan. The report shall include: number of vehicles dispatched; number of scheduled medical trips; number of unscheduled trips; number of trips associated to social events or of a personal nature, number of trips not completed or delayed with rational/remediation plan; number of late pick-ups, and number of incorrect pick up locations.

The Contractor shall be required to provide, operate and maintain a Shuttle Bus Service.

1. The Contractor shall provide a bus schedule.

1. The Contractor shall publish route maps. The maps will be posted at all designated bus stops and provided to the CO.

1. The Contractor shall track patronage by route and provide a report to the CO

1. The Contractor shall provide bus transportation in support of scheduled and unscheduled operations movement.

The contractor shall obtain and maintain Workers Compensation insurance (Defense Base Act) per the FAR 52.228-3, which will be insured at the basic IDIQ award level (not at task order level, due to the time required to get insurance). The DBA insurance will only be required on performance outside the Continental United States (OCONUS) for defense related services. The contractor shall obtain and maintain Insurance-Work on a Government Installation, per FAR 52.228-5: Workers compensation insurance or equivalent workers compensation coverage, as required or prescribed by law, with minimum employer liability coverage of $100,000.00 and general liability insurance covering bodily injury liability insurance coverage of not less than $500,000.00 per occurrence. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. Proof of insurance must be provided prior to award. The insurance will be reimbursable under the ODC CLIN. The Contractor shall provide the CO directly after contract award, a copy of the insurance documents.

19.3 SPECIALIZED MEAL SERVICES

The Contractor may be asked to provide both clinically and culturally appropriate meals for US Government Transported patients, NMAs, and associated family members in accordance with State and Local food safety codes and requirements to the USG transportees at each housing location to include breakfast, lunch and dinner, excluding locations where any of these meal services are provided by the housing provider. In addition, snacks may be required for USG transportees with special medical needs, who receive medical treatment; quantities and time of snack delivery shall be addressed at the task order level. Diet specific meals for medical conditions and portion sizes shall be planned for and approved by a licensed dietician. The Contractor shall provide a sample menu for approval by the COR per each task order. The contractor shall provide a weekly report stating the number of meals delivered and the number of meals not consumed. Per FAR 52.226-6 (In accordance with the Federal Food Donation Act of 2008 (42 U.S.C. 1792), the Contractor is encouraged, to the maximum extent practicable and safe, to donate excess, apparently wholesome food to nonprofit organizations that provide assistance to food-insecure people in the United States.

19.4 COMMUNITY HOUSING

The Contractor shall have the ability to provide housing appropriate to house USG transportees in commercial, community type settings for both short and long term timeframes that are dependent on the mission, or field setting if required. The Contractor may or may not be required to provide the field shelters depending on the emergency. Commercial housing for USG transportees that require special or accessible facilities shall be housed in the appropriate accessible ADA compliant facilities with wheelchair accessible rooms, roll in showers and elevator accessibility, if required. When available, kitchenettes shall be provided in each room to facilitate preparation of meals. Housing is preferred to be at no more than 3-4 separate locations within close proximity of each other to promote a community setting however, the number of locations can be adjusted based on number of USG transportees needing to be housed.

A proposed detailed plan of housing shall be submitted to the COR and CO for approval per task order.

Community Housing Objectives:

· Provide commercial housing in a community type, long-term setting to be billeted in a housing type environment with a kitchenette. In the event USG transportees have to relocate to new facilities once established in a location, the Contractor shall be responsible for the relocation of the USG transportees and their belongings to the new location, (i.e., moving trucks/movers/transportation, etc.).

· Provide appropriate accessible or ADA compliant rooms for USG transportees with special needs such as with wheelchair accessible rooms, roll in showers elevator accessibility.

· Provide housing within 25 miles of social services or hospital treatment centers identified in Places(s) of Performance at the task order level.

· Provide housing in close proximity (<5 Miles) of shopping centers, pharmacies, and locations that provide basic human needs and resources.

· Provide the COR a weekly report that includes the facility location per task order, quantity and type of room at each location and occupied or unoccupied status of each room.

19.5. BILLETING FOR RESPONDERS

The Contractor shall provide, operate, maintain, and manage billeting (housing) for responders, health care workers, deployed camp staff, visitors, and US Government Transported patients, NMAs, and associated family members if commercial housing is not available.

1. The Contractor shall construct temporary housing which would be detailed in the task order SOW, via tents etc., not construction.

1. The Contractor shall conduct cyclic inventories and report discrepancies to the CO.

1. The Contractor shall establish and perform Preventive Maintenance, Checks and Services (PMCS) to ensure billets are maintained and serviceable. Contractor decisions to repair or replace facility equipment identified on the SOW shall be determined by which approach provides the most economy and best value to the government on a case-by-case basis.

1. The Contractor shall provide furnishings as listed: environmental control units (heating and air conditioning), interior and exterior lighting, trashcans, fire extinguishers, smoke alarms and electrical outlets.

1. The Contractor shall provide a linen exchange service for sheets and blankets.

19.5.1 Permanent and Transient Accommodations.

19.5.1(a) Billeting. The Contractor shall operate and maintain billeting. The billeting shall include providing a bed with mattress, night stand w/ lamp, one set of linens consisting of one sheet and one mattress cover and one pillowcase; one pillow, one blanket, window curtains or blinds and one wall locker per individual.

19.5.1(b) Transient Billeting. The Contractor shall operate and maintain billeting for transient population. The billeting shall include providing a cot, one set of linen, two blankets and a pillow per individual. Housing can include a bunk bed, chair, footlocker, two sets of linen (2 sheets and 2 mattress covers/2 pillow cases), two pillows and two blankets.

19.5.1(c) Distinguished Visitor (DV)/VIP. The Contractor shall maintain DV/VIP quarters, to include cleaning, maintenance, and linen provision.

1. The Contractor shall provide daily cleaning of hallways and latrines.

1. The Contractor shall clean the rooms when occupant checks out.

19.5.1(d) OSHA requirements. The Contractor shall meet OSHA Temporary Labor Camp Standard 1910.142 for all billeting related tasks.

19.6 CATERING DINING FACILITIES

At the direction of the Government CO, the Contractor shall perform Food Service Operations and operate Dining Facilities.

19.6.1 Food Service Preparation. The Contractor shall provide, prepare and serve meals.

a. The Contractor shall provide production schedules and food requests.

b. The Contractor shall ensure food safety and sanitation standards are maintained in dining facilities and food storage areas.

(1) The Contractor shall assign and inspect their food service personnel for illness, wear of appropriate protective gear and sanitation.

(2) The Contractor shall comply with Hazard Analysis Critical Control Point (HACCP) standards to ensure food safety as developed by the National Advisory Committee on Microbiological Criteria for Foods.

(3) The Contractor shall provide sufficient hand washing stations using either potable water or hand sanitizer to accommodate the site population.

(4) The Contractor shall provide bottled water.

c. The Contractor shall provide ice machines as required. Be able to provide or generate up to 10,000 lbs a day.

d. The Contractor shall prepare and serve meals for special events when directed by the CO. Special meal requests must be approved by the CO.

19.6.2 Meals Ready to Eat. At the discretion of the Government, the Contractor shall provide Meals Ready to Eat (MREs) for responders, US Government Transported patients, NMAs, and associated family members and facility Government staff.

19.6.3 Convoy Café. At the discretion of the Government, the Contractor shall operate and maintain Convoy Café’s that allows CAC/PIV holders, Government personnel and Foreign National truck drivers to eat.

a. The Contractor shall provide culturally competent meals that may deviate from the standard menus.

b. At the discretion of the Government the Contractor may be required to provide takeout meals.

19.6.4 Ice Production and Distribution. The Contractor shall provide, operate, and maintain ice production equipment.

a. Supply Reserves. The Contractor shall maintain ice reserves.

b. Production. The Contractor shall produce, store and issue ice.

c. Testing. The Contractor shall perform random tests of ice produced in their ice production plant. The Contractor shall ensure potable water used for the production to ensure ice meets or exceeds quality local and Federal standards.

d. Distribution. The Contractor shall operate and maintain ice distribution points. The Contractor shall issue ice from the distribution points to all approved customers.

19.7 LAUNDRY SERVICES (LINEN AND STAFF MEDICAL CLOTHING)

19.7.1 Laundry Operations. The Contractor shall perform laundry operations for community housing occupants as stated in section 19.7 and as detailed within this section as directed by the Government in the task order SOW.

a. Clothing and equipment received for cleaning will not be allowed to leave the Contractor’s immediate control.

b. The Contractor shall process laundry claims and provide a report of claims to the CO.

c. The Contractor shall process lost and found items.

d. The Contractor shall provide laundry drop off/pick up points. The Contractor shall not receive bulk laundry at laundry drop off/ pick up points. The contractor shall provide laundry bags.

19.7.2 Specialized Laundry Service. The Contractor shall segregate clothing or linen soiled with industrial and pesticide products.

19.7.3 Medical Laundry Service. The Contractor shall separately process laundry used in support of a medical facility.

19.7.4 Self-Service Washers/Dryers. At the discretion of the Government and as detailed in the task order SOW, the Contractor shall provide and/or maintain self-service washers/dryers as well as the facilities housing the self-service laundry.

19.7.5 Receiving, Staging, Onward Movement and Integration (RSOI) Laundry. The Contractor shall provide 24-hour turn-around laundry service for RSOI personnel at sites where laundry services are provided by the Contractor.

19.7.6 Sewing. The Contractor shall perform minor clothing repair for uniforms and civilian clothing only if such repair can be made without further damaging the item. Tailoring is not authorized.

19.8 TOILET AND HAND WASHING FACILITIES AND MOBILE SHOWER FACILITIES

The Contractor shall have the ability to provide portable rest room trailers and/or portable toilets and hand washing stations. One toilet per 15 camp inhabitant per site of performance shall be the ratio for use in accordance with OSHA 1910.142(d)(5). These units shall be designed and certified for use by the Portable Sanitation Association International. All units shall be fully serviced once daily, or as site capacity requires, or as directed by the COR. All waste water if generated shall be collected with vacuum trucks and transported offsite and disposed of through legally permissible methods. When available, public utilities shall be utilized. Restroom facilities must be ADA compliant. The Contractor shall ensure the toilets are adequately stocked with toilet paper.

The Contractor shall furnish sufficient hand-washing stations adjacent to the restroom facilities for US Government Transported patients, NMAs, and associated family members to use to maintain proper hand hygiene. The Contractor shall ensure the hand wash stations remain stocked with water, hand soap and paper towels and cleanliness maintained at all times.

The Contractor shall provide mobile shower facilities, 1 showerhead per 10 persons in accordance with OSHA Standard 1910.142(f)(1)(ii). Shower facilities shall have individual stalls. Units shall have cold and hot water. Heated water shall be provided at a minimum of 101 degrees Fahrenheit, with an anti-scald valve for user safety. All showers shall be sanitized daily with a disinfecting solution, when occasionally disinfecting is needed more than once a day, the contractor will be notified. Toilet, hand washing and mobile shower facilities must be ADA compliant.

If mobile shower facilities are provided, the Contractor shall ensure that wastewater shall be collected and disposed of in an appropriate manner compliant with federal regulations. Disposal trucks shall maintain a service schedule to remove the wastewater from the site and transport offsite to dispose of at a permitted treatment facility. Local utilities shall be used if available.

19.9 HYDRATION STATIONS

The contractor shall provide up to 1500 deployed personnel readily available water throughout the site of operations in the form of water dispensers and cups or bottled water. The contractor shall provide 35 gallons of water/person per day in accordance with OSHA Temporary Labor Camp Standard 1910.142(c)(2).

19.10 MATERIAL HANDLING EQUIPMENT (MHE) (FORKLIFTS)

19.10.1 The Contractor shall operate and maintain MHE operations.

1. The Contractor shall perform transportation movement within the confines of the installation and provide all MHE and transportation assets unless those assets are provided by the government.

1. The Contractor shall provide licensed personnel for all MHE equipment that requires an operator license. All certifications shall be current and IAW Occupational Safety and Health Administration (OSHA) standards.

1. The Contractor shall provide and maintain routine inspection documentation on all MHE and Vertical Handling Equipment (VHE) IAW the manufacturer’s guidelines and applicable TMs.

19.10.2 The Contractor shall also provide several (up to 50 types of mixed equipment) in different sizes of forklifts including straight mast rough terrain forklifts, industrial warehouse forklifts, shooting boom rough terrain forklifts, pallet-jacks. Additionally, equipment should include options for a range of box trucks with lift gates, generators with trailers (11 KW – 100 KW), portal HVAC (1T – 5T), standard trucks and vehicle for movement of supplies, ATV vehicles, hand trucks, warehouse shrink-wrap machines refrigerated trucks. Equipment field maintenance and (re)fueling support as requested shall be provided.

19.11 POTABLE/NON POTABLE WATER

The Contractor shall provide, operate, maintain, purify, store, and distribute potable and non-potable water.

1. The Contractor shall provide, operate and maintain Reverse Osmosis Water Purification Units (ROWPUs) at water production locations or other water processing equipment as applicable.

1. The Contractor shall store potable and non-potable water. The storage can vary 10% to account for population variations and incidental usage. Potable water shall meet or exceed quality standards.

1. The Contractor shall be responsible for testing chlorination of the water from all water storage points including various well points, to ensure that water is suitable for personal hygiene.

19.11.1 Local Municipal or Host Nation (HN) Water Works. The Contractor shall report and suggest repairs to and maintain the current local and/or Host nation (municipal) city water works inside the site perimeter. Repairs to the local or Host nation water system inside the base perimeter shall be made on a case-by-case basis. Repairs or maintenance water systems outside the base perimeter shall be reported to the CO/COR. Repairs to the water system outside the base perimeter shall be made by the HN municipality and coordinated by the onsite Federal liaison and ACO. Repairs or maintenance of local and/or Host Nation water systems whether it is inside or outside the site perimeter is considered new work and shall be accomplished through a task order modification initiated by the Federal Government COR and CO.

19.12 GRAY WATER DISPOSAL

Following Federal and local regulations, the Contractor shall provide and develop a plan for Gray water disposal which limits demand of sewage systems managing black water.

19.13 WASTE REMOVAL

The Contractor shall operate and maintain waste management systems.

19.13.1 Dumpsters and Trash Removal. The Contractor shall process waste for removal to authorized trash sites per local laws. Cat A/B waste management is a requirement. Category A infectious substance: An infectious substance in a form capable of causing permanent disability or life-threatening or fatal disease in otherwise healthy humans or animals when exposure to it occurs. An exposure occurs when an infectious substance is released outside of its protective packaging, resulting in physical contact with humans or animals. See 49 CFR § 173.134(a)(1)(i). Note that Category A infectious substances described in this document and covered by the Hazardous Materials Regulations (HMR) at 49 CFR parts 171-180 should not be confused with the select agents regulated by the Federal Select Agent program under 7 CFR part 331, 9 CFR part 121, and 42 CFR part 73 (although an infectious substance or agent may be both covered by the HMR and listed as a select agent). For additional information, see www.selectagents.gov. Category B infectious substance: An infectious substance that is not in a form generally capable of causing permanent disability or life-threatening or fatal disease in otherwise healthy humans or animals when exposure to it occurs. Transport as Biological substance, Category B (UN3373). See 49 CFR § 173.134(a)(1)(ii).

a. The Contractor shall conduct visual safety inspections of dumpsters.

b. The Contractor shall separate hazardous materials and Petroleum, Oil, and Lubricants (POL), and store them in designated locations. Hazardous materials include but are not limited to car batteries, used oil, and industrial chemicals. The Contractor shall transport found hazardous materials to a specified Hazardous Waste Accumulation Point or to a specified Hazardous Waste Storage Area.

19.13.2 Sewage System. The Contractor shall operate and maintain existing sewage system or modular Waste Water Treatment Facilities (WWTF) systems within CAMP site boundaries and additional connections to the sewage system.

19.13.3 Non-Hazardous Waste Management. The Contractor shall conduct Non-hazardous waste management activities as listed below consistent with other environmental provisions of Task Order and the base Contract.

19.13.4. Recycling. The Contractor shall recycle or segregate recyclable materials for disposition.

19.13.5. Incineration. As directed by the CO, the Contractor shall install, operate and maintain Government furnished incinerators.

19.14 SECURITY (FORCE PROTECTION)

Mirroring ESF 13 functions, the contractor shall provide general and specialized security force protection to support stages operational areas during disasters which includes:

· Protect critical infrastructure during prevention activities or disaster response.

· Protect emergency responders.

· Manage the development of pre-scripted mission assignments to address known and anticipated disaster response public safety and security short falls within the area of operation.

· Give priority to life safety missions first, followed by missions that address security and the protection of infrastructure/property.

· Electronic Personnel Tracking: the contractor shall provide electronic means to track Government Employees, patients and contractors on-site of the operational location. The contractor shall maintain a real time data base for personnel while supporting the mission. The tracking device shall be positioned at entrances/exits using passive radio frequency identification (RFID) technology.

· All Protective Security Officers (PSOs) and PSO Supervisors excluding current sworn/certified or retired State and Local Law Enforcement Officers performing under the resulting contract/task orders must meet the following minimum qualifications:

· U.S. Citizenship

· Proficiency in English (written and spoken)

· Current or prior security experience

· Firearms permits

· Have state/local licenses, permits, registrations, or certifications as required

· Must be at least 21 years of age

· The Contractor shall obtain all licenses and permits required for each PSO and supervisor to serve as PSOs as required. PSOs must carry their firearm license/permits (and, where legally required, their concealed weapons permits) on their person while on duty, unless local or state law requires the Contractor to maintain the records. Failure by a PSO to carry a valid firearm certificate or permit while on duty shall result in the PSO being removed from the post until the certificate or permit is obtained. Current sworn/certified or retired State and Local Law Enforcement Officers hired to perform PSO duties in accordance with this Contract are exempt from this requirement.

· No employee of the Contractor shall provide more than twelve (12) hours of combined service on any one or multiple contracts in any twenty-four (24) hour period, unless the work periods are separated by an eight (8) hour non-duty period.

· The limitation on hours may be verbally waived by the COR in emergencies that are beyond the control of the Contractor (i.e., weather conditions that prevent the next shift from getting to the facility, civil disturbances, natural disasters, emergencies, etc.). The Contractor will provide the COR the written waiver request within 1 hour of identification of pending need. The COR will sign and return the waiver to the Contractor.

· The Contractor shall furnish and maintain in acceptable condition all items of uniform and equipment necessary to perform work required by the Contract. The Contractor is solely responsible for the furnishing, quality and performance of all Contractor-provided equipment used in performance of this Contract. The Contractor shall ensure that all uniformed personnel, including subcontractor employees, if any, are uniformed with the prime contractors standard uniform.

· Current sworn/certified State or Local Law Enforcement Officers (LEOs) hired to perform PSO duties in accordance with this contract are exempt from the uniform requirements stated above and may wear their State or Local issued uniform. Retired LEOs must be provided and wear Contractor-Provided uniforms.

· PSOs shall carry Contractor-Provided firearms only, in performance of their duties.

· NO personally-owned firearms are authorized in the performance of this Contract, except those approved by waiver in accordance with this contract, belonging to state or local LEOs who have satisfied qualifications requirements for carry.

19.14.5. All weapons and ammunition utilized under this contract must meet the specifications below.

· The Contractor shall at a minimum develop a Safty Plan and a Management Plan that will detail how they will handle firearms and personnel firearm training.

· Contractor shall provide ammunition for Contractor-provided Firearms. State/Local LEOs may utilize department issued or personally purchased ammunition, as applicable.

· The Contractor, if employing state and local LEOs or retired LEOs, shall request a waiver from the Contracting Officer for LEOs to carry state or local departmental weapons and/or personally owned firearms on this Contract.

· Appropriate and ample supplies of firearms maintenance equipment (cleaning solvents, lubricating oil, rods, brushes and patches, and other normal maintenance tools) shall be provided by the Contractor and at the Contractor’s expense.

· PSOs will use assigned Contractor-Provided firearms and ammunition; one firearm, per PSO. There will be no sharing of firearms, or assigning firearms to a post and not a person. All PSOs shall inspect their assigned firearms at the commencement of each tour of duty. Each firearm shall be cleaned and oiled regularly to ensure optimum operating condition. Firearms shall always be handled in a safe and prudent manner.

· The Contractor must be able to account for all Contractor-Provided firearms at all times.

· The Contractor shall provide a list of serial numbers of all Contractor-Provided firearms that will be carried during this Contract. The list shall be kept current; the Contractor must document and forward anychanges to the COR within one week of the change.

· In the event that a firearm is lost or stolen, the Contractor shall notify local law enforcement as well as the COR and CO immediately relating all the particulars known regarding the loss or theft of the weapon. Additionally, the Contractor shall provide a detailed written report to the COR within 4 hours of the incident, including the date and time of the incident. The Contractor shall also notify the COR of the serial number for the replacement weapon.

· Semiautomatic weapons are the required firearm for contracted PSOs; to include State or Local LEOs hired to perform PSO duties.

· Common law enforcement caliber ammunition will be used, such as 9mm and .40 calibers.

· Contractor shall provide to the PSO and PSO’s shall carry three ammunition magazines, minimum of 10 rounds in each magazine. Each magazine will be carried full of ammunition. Two magazines will be spares and carried on the duty belt in a holder designed for that purpose. One magazine will be inserted in the handgun. The weapon will have a round in the chamber. State/Local LEOs may carry ammunition in accordance with departmental guidelines. Retired LEOs should carry ammunition similar to PSO standards outlined above.

· Revolvers should not be used on this PSO contract unless mandated by State or Municipal statutes and/or specifically approved by the COR.
