Subject: Amendment No. Four (4) to Request for Proposals (RFP) No. 72029423R00006
ATTENTION PROSPECTIVE OFFERERS:
The purpose of this amendment is to make the following changes to the subject RFP. The below link provides access to the Amendment No. 4 atachments referenced hereto:

A. RFP Cover Letter: Delete paragraph four in its entirety and insert the following in lieu thereof:
“Joint Ventures may respond under this RFP. Please refer to Section L which lists the required documentation to be submitted at the time of submittal of the proposal to be deemed eligible. In the absence of any required proof of documentation, the proposal will not be considered for award. “
B. SECTION C - DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
Under Sub-Sec�on ‘C.7.3 Permits and Approvals” delete the contents of the 3rd bullet in its en�rety and insert the following in lieu thereof:
● “A valid registra�on cer�ficate issued by the Israeli Ministry of Construc�on and Housing, and a valid classifica�on from the Local Union of Contractors (Grade Gemel 2 or Gemel 3 in Buildings and Electromechanical work);”
C. SECTION J – LIST OF ATTACHMENTS –
I. Delete the contents of Attachment J-5 and replace with the following ‘Attachment J-5 – PAST PERFORMANCE REPORT Rev.1’ provided under this Amendment.
II. Add the following attachments included under this amendment:

D. SECTION L- INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS
I. L.7 INSTRUCTIONS FOR THE PREPARATION OF THE TECHNICAL PROPOSAL – Delete the contents of part A. “VOLUME I - ORGANIZATION OF IDIQ RFP TECHNICAL PROPOSAL – GENERAL INSTRUCTIONS” in its entirety and insert the following in lieu thereof:
“Offerors must submit a complete technical proposal titled “Volume I: Technical Proposal for Basic Infrastructure Support Mechanism RFP IDIQ.”
The written Technical Proposal for Volume I is limited to 15 pages and must be written in English. USAID must not evaluate information submitted above this page limit. Do not use a type smaller than 11 point font. Offerors must use only 8.5 inch by 11 inch (210mm by 297mm) paper, single-spaced pages with margins no less than one inch on each border. Number each page consecutively. The technical proposal must include the information set forth below.
Note: A page in the technical proposal that contains a table, chart, graph, etc., not otherwise excluded below, is subject to the page limitation.
Not included in this page limitation are the following:
● Title Page
● Cover Letter
● Table of Contents
● Acronym list
● Executive Summary
● Response to Eligibility Requirements
● Annex 1: Management Structure Organizational Chart(s)
● Annex 2: Past performance Information (per Section L and Attachments J-5, J-8, and includes any quality awards or certifications)
● Annex 3: Contractor’s Available Equipment
All information from appendices must be referenced in the technical proposal and summarized and included in the annex section.”
II. Under part B. “CONTENT OF TECHNICAL PROPOSAL – VOLUME I – SPECIFIC INSTRUCTIONS” delete the first paragraph in its entirety and insert the following in lieu thereof:

A Financial Capacity Evidence of
Financial Capacity
The Offeror must show its financial capacity for the work an�cipated under this award during the contract period of performance.
Offerors must demonstrate the ability to achieve the performance and maintenance guarantee/ bonding capacity at a minimum amount of an aggregate capacity of $2,000,000 per year.
In Page 1: Offerors must submit a leter from the Offeror’s surety, from an established bank sta�ng their commitment to bond the Offeror at a minimum of $2,000,000 aggregate per year for the five-year performance period, if awarded a contract.
Note: An actual Guaranty/bond is not required.
B Registra�on and Classifica�on Evidence of

requirement s special�es in three main categories: Building (grade 1-A), Water and sewer, and Electro-mechanical.
Other special�es must be determined and specified, if any, at the Task Order level.)
Note: Only registered and classified contractors must be allowed under this Contract.*

In Page 4: Provide a Valid Contractor Classifica�on Cer�ficate from the Na�onal Classifica�on Commitee - Ministry of Public Works and Housing (MOPWH) confirming that the contractor is classified as Grade 1 in all of the following three categories:
Building (grade 1-A), Water and Sewer, and Electro-mechanical.
*For JVs – one page for each JV member Organiza�on.
* Joint Ventures may respond under this RFP. However, proof of the registered Joint Venture as a local en�ty as well its registra�on in the MONE, the PCU and classifica�on from the Na�onal Classifica�on Commitee - Ministry of Public Works and Housing (MOPWH) is required at the �me of submital of the proposal.
Within a JV, it must be demonstrated that the requirements for a Grade 1 in all three categories iden�fied above are valid. For example, if two organiza�ons are forming a JV, and one organiza�on has a Grade 1 in Water and Sewer as well as Electromechanical and the second organiza�on has a Grade 1-A in Building, that Offeror/JV will be considered eligible. As such, offerors may also submit separate cer�ficates for each organiza�on, on behalf of the JV to demonstrate how they meet this requirement. In the absence of proof of a current registra�on and classifica�on, the proposal will not be considered for award.”
IV. Under Sub-section VI. FACTOR 1: OFFEROR CAPACITY (Reference Section M.2.I) –
Delete part B. in its en�rety and insert the following in lieu thereof:
“Compliance and Performance Assurances
(i) The Offeror must describe its own resources (such as in-house exper�se, policies, procedures, manuals, so�ware, and systems), used to ensure the following:
• Work site health and safety prac�ces and procedures.
• Accurate scheduling and �mely comple�on of mul�ple concurrent construc�on projects in mul�ple loca�ons.
• Effec�ve Quality Control/ Quality Assurance system to ensure full compliance with the contractual design specifica�ons and quality standards for construc�on projects from ini�a�on through comple�on.
• In narra�ve form and suppor�ng tables describe the offeror’s labor and equipment base (type, quan�ty, condi�on, and loca�on).
(ii) The Offeror must describe (in narrative form and supporting tables) what its resources and systems will be used to ensure that reputable and specialized subcontractors and suppliers will be used, and how it must integrate them into its construction team to undertake the construction work required.”
V. Delete the contents of sub-section VIII. “FACTOR 4: PAST PERFORMANCE (Reference Section M.2.IV)” in its entirety and insert the following in lieu thereof:
“Provide an analysis of the complexity of the requirement and indicate how any an�cipated problems must be iden�fied and resolved.
(a) The Offeror (including all partners of a joint venture) must provide performance informa�on in accordance with the Past Performance Matrix and Past Performance Report in Atachments J-5 and J-8 in accordance with the following:
1. List in annex 2 to the technical proposal up to a total of 5 of the most recent and relevant contracts with a minimum value of $1,500,000, currently being implemented or completed during the last seven (7) years for efforts similar to the work described in the Statement of Work in Sec�ons C. At least two of the listed projects must be in building/ver�cal structure. The Past Performance Report must clearly state whether the contract was performed by the Offeror in the capacity of prime contractor or as a subcontractor.
2. For each contract listed above that is not recorded in CPARS, please provide the following details: contact name, job �tle, mailing address, phone number, e-mail address, explana�on of relevance to the proposed acquisi�on and a descrip�on of the performance to include:
• Scope of work or complexity/diversity of tasks,
• Primary loca�on(s) of work,
• Term of performance,
• Dollar value, and
• Contract type, i.e., fixed-price, cost reimbursement, etc.
(USAID recommends that you alert the contacts that their names have been submited and that they are authorized to provide performance informa�on concerning the listed contracts if and when USAID requests it.)
(b) If extraordinary problems have impacted any of the referenced contracts, provide a short explana�on and the correc�ve ac�on taken (FAR 15.305(a)(2)). This informa�on is not included in the page limita�on and must be filled out in the Past Performance Report, Atachment J-5.
Offerors are expected to provide more than just general informa�on on their performance.
Significant problems that must be explained with details include:
1. Cost increases or overruns that were incurred. These problems must be explained in terms of:
● A brief descrip�on of the nature and reason/cause for the cost overrun,
● The cost impact of the cost increase or overrun in terms of:
o The dollar value in excess of the contract amount.
o The percentage value in excess of the contract amount.
2. Performance delays which delayed completion beyond the contractual completion date. These problems must be explained in terms of:
● A brief descrip�on of the nature and reason/cause for the performance delay,
● The schedule impact of the delay in terms of:
o The number of calendar days of delay beyond the contractual comple�on date.
o The dura�on of the delay rela�ve to the overall project length.
3. Quality of work that failed to conform to the contract requirements or was otherwise found to be a defect in equipment, material, or workmanship performed by the Offeror or any subcontractor or supplier at any tier. This includes any warranty work performed under the referenced contract. These problems must be explained in terms of:
I. A brief descrip�on of the nature and reason/cause for the quality problem, II. The dollar value of remedial work (if any) and its percentage value in terms of the contract amount
III. Whether the quality remedy was performed by the Offeror during contract performance, a�er contract comple�on but during the warranty period, a�er the warranty period, or not remedied at all.
4. Customer satisfaction issues resulting in dissatisfactory business relationships with clients, difficulties in the initiation and management of several complex activities simultaneously, challenges in coordination among subcontractors and developing country partners, dissatisfactory correction of problems, whether a cooperative attitude was used in fixing problems.
5. Management and key personnel issues in terms of effectiveness and appropriateness of personnel for the job; whether prompt and satisfactory changes were made in personnel when problems with clients were identified.
Were the contractor’s on-site and home office management personnel functioning adequately in terms of planning, scheduling, allocating resources, organizing and otherwise managing the work? If not, describe the problems faced and the steps taken to resolve the problems.
6. Failure to comply with Safety such as the lack of an effective safety program and effective measures to correct safety deficiencies.
7. Failure to comply with labor standards and any violations of the labor laws and provisions and whether corrective actions were taken to correct them. This includes payroll records being submitted in a complete and timely manner and other labor related issues.
(c) Describe any quality awards or cer�fica�ons that indicate excep�onal capacity to provide the service or product described in Sec�on C. This informa�on must be included in Annex 2.”
E. SECTION M - EVALUATION FACTORS FOR AWARD, M.2 TECHNICAL EVALUATION
CRITERIA
Under sec�on IV. “FACTOR IV: PAST PERFORMANCE – See Sec�on L.7.B.VIII for reference” Delete part (5) under sub-sec�on (d) in its en�rety and insert the following in lieu thereof:
“(5) Regulatory Compliance. (e.g. failure to report in accordance with contract terms and condi�ons, late or nonpayment to subcontractors, trafficking viola�ons, tax delinquency, defec�ve cost or pricing data, termina�ons, suspension and debarments) Assess compliance with all terms and condi�ons in the contract/order rela�ng to applicable regula�ons and codes. Consider aspects of performance such as compliance with financial, environmental, safety, and labor regula�ons as well as any other repor�ng requirements in the contract terms and condi�ons.”
F. RFTOP SECTION J, DOCUMENTS, EXHIBITS AND OTHER ATTACHMENTS
1. Attachment J.1, Task Order Bill of Quantities for Youth Development Resource Centers (Attachment J-3 of the BISM IDIQ RFP)
1.1 Delete the existing Bill of Quantities and replace with the revised Bill of Quantities attached hereto.
2. Attachment J.2.A, Project Specifications (Division 1 – Youth Development Resources
Centers)
2.1 Delete the existing Section 01300 and replace with the revised Section 01300 attached hereto.
