About this file

This is a Past Performance Questionnaire (PPQ) template for solicitation 70Z08525RLREP0002, related to dockside repairs for the USCGC BERTHOLF. The document provides instructions for offerors to submit past performance information, specifying that if a completed CPARS evaluation is available, it should be submitted instead of the PPQ. If no CPARS exists, offerors must have their previous clients complete the questionnaire and submit it directly to Wayne Gutman with a copy to Amy Kelm before the solicitation closing date.

The PPQ template includes rating criteria across three main categories: Quality of Product or Service, Timeliness of Performance, and Business Relations. Each category is rated on a 0-5 scale (Unsatisfactory to Outstanding), with detailed evaluation criteria provided for each rating level. The questionnaire requires basic contract information including contract value, period of performance, and customer contact details. The related contract opportunity indicates the work will be performed at the cutter's homeport in Alameda, CA, with a performance period from February 11, 2025, to April 9, 2025.
