MARKET RESEARCH QUESTIONAIRE
TESTING
1. Is your company capable of performing all the test methods by the standards listed below? If your company cannot perform any of these standards, please mark which ones. If your company performs a different test method for a specific fuel property, please include the standard used.

2. Can your company create fuel testing kits that consists of all the required materials needed for the user to take and ship out five (5) separate fuel samples per kit to the laboratory? This includes plastic bottles/cubitainers, serialized seals, protective packing, bottle labels, pre-addressed shipping mailers with pre-paid courier account numbers, screw caps, and instruction manuals.
3. The fuel samples must be able to be shipped as non-HAZMAT when shipping from the vessel to the laboratory. Will your company’s fuel kit include all the supplies needed to meet IATA, DOT, and other applicable regulations for air and ground shipping fuel samples as non-hazardous material to the Contractor’s fuel laboratory?
4. Does your laboratory use a specific courier service for shipping? If so, which company is used? Some regions only have one courier service option, will your company be able to use a different courier service if needed? This includes shipping the fuel test kits from the lab to the vessel and shipping fuel samples from the vessel to the lab.
5. Does your laboratory have an online database that sample results will be uploaded to?
6. Can your laboratory run all the tests listed in question 1 with a turnaround time of three (3) business days of sample receipt at the Contractor’s laboratory?
7. In the SOW, it is required that only one laboratory location will be utilized to test all fuel properties. Does your company have a single laboratory location that can perform every test in question #1 of this questionnaire?
8. Does your laboratory have the capacity to process a minimum of ten (10) fuel samples per day?

9. How does your company verify accuracy of test results?
10. What type of a QA program does your company have in place?
11. Please provide your company’s ISO certification if any.
12. Would your company allow a customer to conduct an audit of your facility?
PRODUCTION
13. How many fuel samples does your company test annually? Can your company handle an additional 800 fuel samples annually while maintaining normal efficiency?
14. What is your company’s normal turn-around time for processing and reporting a fuel sample result? U.S. Coast Guard’s current requirement is 72 business hours.
DATA ANALYSIS
15. Is data interpretation/analysis provided free of charge or part of the sample processing cost?
16. What type of analytical method does your company employ on data interpretation?
DATA DELIVERY
17. What are your company’s methods of electronic data delivery to your customers? This is regarding individual sample results and for larger data exports from all or part of a complete historical database of samples.
18. Does your company use a File Transfer Profile site to transfer data to the customer?
19. What file type is your company able to send for both fuel sample results and for any data pulls requested. Please provide format copy of electronic data delivery if possible.
DATABASE
20. Does your company provide access to your online web-based database to your customers free of charge or with additional fee? Does your online database have the capability to provide comments and recommendations, distribute summary reports via email to shipboard and shore side personnel, print reports, and export data?
