About this file
This is a Past Performance Form template for contractors responding to DHS solicitation 70RTAC26R00000004 for the Department-Wide Cloud (Cumulus) Multiple Award contract. The form serves as a standardized template requiring offerors to document their relevant contract experience and demonstrate past performance capability.
The form requires contractors to provide comprehensive information about previously completed or ongoing contracts, including the customer organization and address, contract number, award date, total project price, period of performance, security clearance requirements, level of effort in hours per year, number of employees assigned, and primary technical and contracting points of contact. Contractors must describe the geographic location of work performance and indicate whether a Contractor Performance Assessment Report (CPARS) is finalized and resident in SAM.gov. A critical section requires detailed narrative description of contract work scope, magnitude, and complexity with specific comparison to the Cumulus solicitation requirements, including documentation of any unique requirements, performance problems, Department of Labor investigations, and corrective actions taken. Additionally, contractors must disclose whether the previous contract required a subcontracting plan and provide details on subcontractor names, roles, and extent of work performed. This template is designed to enable the government to evaluate contractor experience and performance history relevant to providing cloud services (IaaS, PaaS, SaaS, professional services, marketplace solutions, and training) under the IDIQ contract vehicle.
