About this file

This is a Trench Safety Act Requirement form for Broward County's Broadview Park Drainage Green Infrastructure Project, which involves furnishing all labor, materials, equipment, supervision, and incidentals for roadway and drainage components, maintenance of traffic, resurfacing, pervious asphalt, pervious concrete, pavement markings, barrier wall, stormwater pump station, and landscaping within the Broward County Municipal Service District. The form must be returned with the vendor's submittal or submitted within three business days of the county's request, with failure to comply potentially resulting in the vendor being deemed nonresponsive.

The form addresses compliance with Florida's Trench Safety Act (Sections 553.60-553.64, Florida Statutes) and OSHA excavation safety standards (29 C.F.R. Section 1926.650) for any project involving trenches exceeding 5 feet deep. Vendors must affirm their awareness of the Act and commit to full compliance with applicable trench safety standards, including special shoring requirements, with this assurance being legally binding on all employees and subcontractors. The form requires vendors to identify anticipated methods and costs of compliance, providing pricing tables for either open-end or fixed contracts with units measured in linear feet for standard work and square feet for special shoring, though no specific funding sources or contract amounts are specified in this requirement document.
