About this file
This is a Past Performance Questionnaire document for Manor Independent School District (MISD) in Texas related to RFP 26-004RFP for Virtual and Hybrid Learning services. The questionnaire is designed to be completed independently by client references for proposers responding to the RFP. Offerors must submit a questionnaire package for each project listed as a reference and are responsible for ensuring their client references complete and submit the forms directly to Chrissie Bryant, Purchasing Coordinator, at chrissie.bryant@manorisd.net by Monday, February 23, 2026 at 2:00 PM (CT). References must be submitted on company letterhead from a corporate email account. Proposers are required to provide a minimum of three preferred references from current customers outside of Manor ISD, including Educational Agencies (K-12 preferred), Government Agencies, and firms substantially serviced by the proposer, with references most similar to Manor ISD prioritized.
The questionnaire evaluates proposer performance across multiple dimensions including service type provided, work timeline, quality of services and goods, alignment with expectations, timeliness of completion, and overall contractor performance regarding requirements, schedule, and price. References rate performance using five assessment levels: Exceptional (exceeds most contractual requirements with few minor issues), Very Good (exceeds some requirements with minor issues and effective corrective actions), Satisfactory (meets contractual requirements with minor issues and effective corrective actions), Marginal (meets some requirements with significant problems and somewhat effective corrective actions), and Unsatisfactory (does not meet contractual requirements with serious problems and ineffective corrective actions). The questionnaire also includes a Not Applicable option for performance information deemed not recent or relevant and allows references to provide explanatory narratives and general comments regarding contractor performance.
