About this file
This document is a Litigation History Form for a state and local contract opportunity issued by Broward County, Florida, specifically for the Port Everglades Department. The contract is an open-end solicitation for Underwater Diving Services, requiring a dive team with or without a boat to perform services for Port Everglades and potentially other Broward County agencies. The form outlines comprehensive disclosure requirements for vendors to report any material legal cases from the past three years, including litigation, arbitration, or administrative proceedings that might impact their ability to perform contract work.
The solicitation includes procurement preferences for Small Business Enterprises (SBE) and County Business Enterprises (CBE), with vendors required to submit responses electronically through the BPRO system. Vendors must disclose any significant legal cases involving their organization, including those related to similar work, allegations of fraud or negligence, contract performance issues, financial conditions, or criminal proceedings. Failure to fully disclose litigation history or comply with submission requirements may result in the vendor being deemed non-responsive. The form provides a detailed template for reporting legal cases, with specific requirements for documenting case details, dispositions, and potential impacts on the vendor's contractual capabilities.
