About this file

This document is a Contract Exceptions Form for Request for Proposal (RFP) #269.2026.146. The form provides a template for potential vendors to list any exceptions or proposed changes to the terms and conditions of the RFP and its associated Sample Contract. The purpose of this form is to allow proposers to formally document any deviations from the original contract specifications while understanding that material changes may result in proposal rejection.

The form requires proposers to specify the RFP/Contract section number, section title, and provide a detailed explanation of any exceptions or proposed changes. At the bottom of the form, there are spaces for the date, company name, signature, and printed name and title of the authorized signatory. The document emphasizes that all proposed exceptions will be carefully reviewed, and any changes that significantly alter the proposal terms may lead to disqualification from the bidding process.
