About this file

This is a Federal Bureau of Prisons Service Contract Business Management Questionnaire template designed to evaluate qualified organizations seeking to provide services to the BOP.

The questionnaire requires organizations to submit comprehensive information across multiple categories. In the Instructions for Filing section, respondents must identify the specific solicitation or requirement, provide legal organization name and business address with contact principals, detail current personnel organized by discipline at the performance office, identify all proposed subcontractors and team members with their areas of responsibility, and list the five most recent and relevant contracts performed within the past three years. For each contract, detailed information is mandatory including project name and location, services description, contract number, contract type, work complexity level, customer contact information, contract value, status, award and completion dates, subcontracting details, and Contracting Officer's Representative contact information. The Organizational Information section requires documentation of incorporation details, licensing and registration across jurisdictions, and organizational structure based on entity type (corporation, partnership, or individual ownership). The Experience section addresses claims and litigation history, failed contracts, pending judgments or arbitration, principals' involvement with failed organizations, bankruptcy filings within seven years, and Federal contracts performed in the past five years. Financial capability must be demonstrated through recent audited, reviewed, or CPA-prepared financial statements including balance sheets, income statements, and cash flow statements, along with trade and bank references. All information must be certified as true, accurate, complete, and not misleading, with warnings regarding criminal penalties under 18 U.S.C. § 1001 for materially false statements.
