About this file

This is a Conflict of Interest Disclosure and Certification form for the Town of Ellington, Connecticut, related to a Request for Goods procurement for Senior Center Vehicles (Bid #2026-09-ITB-017). The form must be completed and submitted with all bid or proposal responses. The procurement seeks electronic bids for the purchase of two new vehicles for the Ellington Senior Center Transportation Program: one ADA-accessible wheelchair vehicle and one gasoline-powered crossover SUV. Bidders may submit pricing for either or both vehicles, with each vehicle to be evaluated and awarded separately in accordance with the specifications and contract documents provided.

The Conflict of Interest Disclosure and Certification form requires vendors and bidders to disclose any financial interests, personal relationships, employment relationships, or other affiliations involving the vendor, its owners, officers, directors, employees, subcontractors, consultants, or immediate family members that could create an actual or perceived conflict of interest. Vendors must also disclose if any principal of the vendor holds an elected Town office, appointed Town office, or Town employment status, and must list any current or pending contracts, consulting agreements, business relationships, or other arrangements with the Town relevant to the procurement. The form includes a certification statement made under penalty of false statement pursuant to Connecticut General Statutes 53a-157b, and vendors agree to promptly notify the Town in writing if a conflict of interest arises during the procurement process or contract term. Non-disclosure or false information may result in disqualification from the procurement process, contract termination, or other remedies permitted by law.
