About this file

This is a Confidential and Proprietary Information Declaration Form issued by Salt Lake City Corporation Purchasing & Contracts Management Division for use by contractors and private providers responding to Request for Proposals SLCI26092 for Agenda and Minutes Software. The form enables bidders to identify and protect trade secrets and commercial financial information in their submissions in accordance with Utah Code 63G-2-305 and 63G-2-309. Contractors must submit confidentiality claims concurrently with the protected information and provide a concise statement of reasons supporting each claim, identifying specific pages and paragraphs that contain sensitive data.

The form requires claimants to provide company information, contact details, and specify the governmental entity receiving the submission, along with reasons for the confidentiality request. Notably, submission of a confidentiality claim does not guarantee protected classification status if the governmental entity receives a public records request. Bidders have the option to provide a redacted public copy of their proposals alongside the confidential version. The form includes designated fields for documenting up to four separate confidentiality claims with corresponding page numbers, paragraph references, and supporting explanations.

Business Request for GRAMA Business Confidentiality Claim For use by contractors or private providers to identify protected records or information
