About this file

This document is an Attachment A Small Business Subcontracting Plan form from the Virginia Department of Small Business and Supplier Diversity (DSBSD), which requires potential offerors to complete and submit the document with their proposal to be considered responsive. The form is designed to capture information about subcontracting plans, with a specific focus on utilizing DSBSD-certified small businesses, including women-owned, minority-owned, and service-disabled veteran-owned businesses. The document provides space for offerors to detail up to five potential subcontractors, including their company names, contact information, certification numbers, value percentages or dollar amounts for the initial contract term, and descriptions of work.

The plan defines a small business as one certified by DSBSD, with certification applications available online. The form emphasizes that proposed subcontractor participation will be incorporated into the subsequent contract and become a contractual requirement, with failure to obtain the proposed participation potentially resulting in a breach of contract. The document instructs offerors who are already DSBSD-certified as a micro/small business to complete Section A, while those planning to use subcontractors must complete Section B, detailing their intended utilization of DSBSD-certified small businesses in joint ventures, partnerships, subcontracting, and supply chain roles.
