About this file

This document is a Conflict of Interest Questionnaire (Form CIQ) provided by the Texas Ethics Commission for vendors doing business with local governmental entities. The form is designed to be filed in accordance with Chapter 176 of the Local Government Code, specifically when a vendor has a business relationship with a local governmental entity as defined under Section 176.001(1-a). The questionnaire must be filed with the records administrator of the local governmental entity no later than the 7th business day after the vendor becomes aware of facts requiring the statement to be filed.

The form requires vendors to disclose any employment or business relationships with local government officers or their family members that could potentially create a conflict of interest. This includes reporting any taxable income exceeding $2,500 in a 12-month period or gifts with an aggregate value of more than $100. The purpose is to ensure transparency and prevent potential conflicts in government contracting by requiring vendors to reveal any significant financial connections or gifts to local government officials. Vendors who knowingly violate these disclosure requirements commit a misdemeanor offense under local government code.
