Attachment L4 – Past Performance Questionnaire

Volume IV, Section 1: List and Description of Past Performance References

Instructions to Offerors:

Offerors must complete a submit a list and description of past performance references for three

(3) contracts in accordance with Section L.14.1. The Offeror must submit the list and description of past performance references as Volume IV, Section 1 of the proposal by the date and time identified in Section L.3. If any of the references cited in Volume IV, Section 1 pertain to non- Federal contracts, the Contractor must submit an authorization to release information for that contract (Attachment L5) as part of Volume IV, Section 1.

Volume II, Section 2: Past Performance Questionnaire

Instructions to Offerors:

The Offeror must provide the Past Performance Questionnaire (PPQ) to appropriate customer points of contact for the three past performance references listed in Volume IV, Section 1 of its proposal.

• For Government references, the Offeror must provide the PPQ to two (2) references (the Contracting Officer (CO) and an individual from the Program Office). The Program Office reference should be an individual with firsthand knowledge of the Offeror’s performance such as a Contracting Officer’s Representative (COR) or a Task Order Manager or Representative.

• For past performance under other contracts, such as commercial contracts, the Offeror must provide the PPQ to individuals with duties like those of the Government CO and COR.

To aid the person completing the survey, the Offeror must complete Block 1 of the PPQ.

The Offeror is responsible for following up and encouraging references to submit the PPQ. The due date for the completed PPQs is the same as the due date for all proposal volumes identified in Section L.3. The individual completing the survey (not the Offeror) must submit the completed the survey via e-mail to the FAA CO identified in Section L.8 of the EPICS II SIR.

Instructions to the Individual Completing the Past Performance Questionnaire (PPQ):

To assess Offerors responding to the Federal Aviation Administration’s (FAA’s) Screening Information Request (SIR) for Enterprise Programs, Infrastructure, and Communications Services II (EPICS II), the FAA is seeking past performance information with respect to each Offeror. You have been selected as a past performance reference by one of the Offerors. The name of the Offeror, contract title, and contract number are identified on the next page under Block 1. Within the PPQ, the Offeror is referred to as “the Contractor” relative to its support of your organization. Please do the following to complete and submit the PPQ:

• Correct any information in Block 1 that is inaccurate.

• Complete Block 2

• Complete Blocks 3a – 3f of the PPQ.

o The PPQ is predominantly in a multiple-choice format. Narrative explanations are only required when a rating of “Marginal” or “Unsatisfactory” or “No” is indicated.

o Enter all narrative explanations directly in to the PPQ form.

• Sign Block 4 (scanned wet ink signatures or electronic signatures are acceptable)

• Email the completed PPQ and e-mail to Alessha.Mason@faa.gov with a copy to

Elizabeth.H.Williams@faa.gov no later than TBD.

o The subject line in the submission e-mail should clearly indicate: EPICS II Past-

Performance Questionnaire Submission for <Contractor’s Name>.

o The e-mail message must originate from your government or corporate e-mail address, as applicable.

You may receive a follow-up phone call or e-mail from the Contracting Officer to confirm or clarify information.
