Amendment No. 2 to
DOT Mentoring Solicitation No. 693JK424R500007
This Amendment No. 2 answers a further question posed by a prospective offeror.
6.Question: Our company is having difficulty in extracting a clean free-standing copy of the Solicitation’s Attachment J-2 (Past Performance Questionnaire Form). So could DOT please provide one? In addition, the instructions say to have the Past Performance Questionnaire form submitted with the proposal, yet the form indicates that it is confidential. Please advise.
6. Answer: See the clean and revised free-standing Past Performance Questionnaire Form pasted in Word between horizontal lines at the very bottom of this Word-file-Solicitation-Amendment No. 2. The confidentiality language previously set forth at the bottom of that Past Performance Questionnaire Form has been deleted, by DOT’s contracting officer, from the revised Past Performance Questionnaire Form set forth below. No Past Performance Questionnaire Form, completed by a customer of an offeror company, for use by that offeror in its proposal submitted to DOT in response to this Solicitation, will be unviewable by that offeror-company, since that offeror-company will have possession of that completed Form and may examine it in order to decide whether or not to include it in the offeror’s proposal submitted to DOT. Therefore, this Solicitation Amendment 2 negates, expunges and over-rides any Past-Performance-Questionnaire-confidentiality-language to the contrary given in this Solicitation’s initially-published Past Performance Questionnaire Form, or given anywhere else in the initially-published Solicitation.
This Amendment No. 2 makes no other changes to the subject Solicitation, and provides no other clarifications concerning it.
