About this file

This is a Post-Abatement Verification Statement issued by Environmental Testing & Consulting, Inc. for the State of Michigan regarding hazardous materials abatement at 668 W Hollywood, Detroit, Michigan 48203. The document confirms completion of abatement and removal services for regulated asbestos-containing materials and other hazardous materials identified at the property. The post-abatement inspection was conducted on January 15, 2026, with the original report dated November 26, 2024 and a revised report date of February 4, 2021. Inspector Randy Keathley certified that all safely accessible regulated materials have been abated or removed, and the site has passed visual inspection and is cleared for demolition. Abated quantities included 925 linear feet of exterior caulk, 8,725 square feet of plaster, 100 square feet of additional plaster, 2 square feet of stack cement, 5 square feet of heat shield, 2 natural gas meters, 1 security system, 1 refrigerator/freezer/blast chiller unit, and multiple paint cans and smoke detectors.

Materials remaining on site total 199 square feet of plaster and roofing material, which remain due to unsafe conditions and inaccessible areas where structural compromise poses collapse risk. The abatement contractor MWV Environmental Services, Inc., supervised by Juliette T. Brooks (Accreditation Number A59120), is responsible for removal and transport in accordance with applicable federal regulations. The document recommends that demolition activities treat all remaining debris as asbestos-containing waste material per 40 CFR 61.145(c)(iii) and 61.150, and comply with worker protection requirements under 29 CFR 1926.1101. Date-and-time-stamped photographs documenting material conditions are attached as supporting documentation.

KEY / GUIDANCE ON COMPLETING THE POST-ABATEMENT VERIFICATION STATEMENT

3. The estimated quantities of accessible regulated asbestos–containing or other hazardous/regulated materials that were identified in the report for this site but have NOT been abated or removed prior to this inspection. These materials are grounds for failing the site. The Inspector should attach a date-and time-stamped picture of the remaining materials.

4. The estimated quantities of regulated asbestos-containing or other hazardous/regulated materials on this site that cannot be abated due to safety/accessibility concerns. The material(s)/quantities included under this column were identified in the report for this site or during the post-abatement inspection but cannot be abated/removed prior to demolition due to safety/accessibility concerns. Inaccessible areas or materials that are unsafe to abate/remove/inspect are NOT grounds for failing the site.

5. These tables lists ALL suspect or presumed asbestos-containing materials or other hazardous/regulated materials that were identified during the post-abatement inspection of this site.

7. The estimated quantities of accessible suspect or presumed asbestos-containing materials or other hazardous/regulated materials that were identified during the post-abatement inspection and were immediately abated/removed by the abatement contractor.

6. The estimated quantities of suspect or presumed asbestos-containing or other hazardous/regulated materials that were identified on site during the post-abatement inspection. These materials were NOT identified or listed in the most current report for this site. Suspect or presumed materials must be sampled and analyzed or abated in accordance with the applicable Scope of Services. If the abatement contractor elects to sample and analyze the suspect material, then the assigned due diligence contractor must prepare and submit a revised report as directed by the Owner. If the results determine that the material is NOT a regulated asbestos-containing material, then the post-abatement inspector may pass the site. If the results determine that the material is a regulated asbestos-containing material, then the post-abatement inspector must fail the site and await a subsequent request for a post-abatement inspection.

2. The estimated quantities of regulated asbestos-containing or other hazardous/regulated materials that were listed in the most current report for this site.

1. These tables lists ALL regulated asbestos-containing materials or other hazardous/regulated materials that were identified in the most current report for this site.
