About this file

This is a notice of identification card process and required contractor documentation for Project 620-23-206, which involves renovating Building 14CD for a homeless program at the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The document outlines mandatory security and background screening procedures that all contractor employees and subcontract employees must complete within five business days of contract award before commencing work.

The process requires contractors to submit completed OF-306 Contractor Request Forms and self-certifications for each employee to their assigned Program Office Representative or Program Manager, using secure transmission methods to protect personally identifiable information such as social security numbers. Additionally, each contractor employee must visit a VA facility with two forms of identification to provide fingerprints and complete a Fingerprinting Request Form, which determines billing and result routing. The WMC HROO PerSec Contractor Team screens all submissions and fingerprint results using the National Criminal History Check form to issue favorable or unfavorable screening decisions. The notice emphasizes that any contract personnel substitutions or additions require completion of all required documentation and receipt of a favorable screening decision prior to beginning work. The effective date is March 14, 2025, and this is Revision 04 of the notice.
