About this file

This is a Proprietary Information, Qualification Questionnaire, and Mandatory Requirements document (RFQt-06) for RFQt #25-0227 issued by the City of Fort Worth Purchasing Division for Scantron products and services. The document serves as a qualification questionnaire that proposers must complete as part of their proposal submission to be considered responsive. The questionnaire covers various aspects including cooperative agreement availability, prompt payment discount terms, catalog pricing discounts for additional items, company business history and locations, email responsiveness, business hours, and whether proposers made changes to the draft vendor service agreement.

The document establishes mandatory submission requirements including completed forms RFQt-02 (Form CIQ), RFQt-03 (Authorized Signed and Requirements), RFQt-06 (Questionnaire and Requirements), signed addendum acknowledgments, and a bid table. The questionnaire includes provisions for proprietary information protection under the Texas Public Information Act, with specific marking requirements using "PROPRIETARY" in yellow highlighting, though pricing information cannot be marked as proprietary. The document references Exhibit A and the RFQt-07 Draft Vendor Service Agreement as components that proposers must review and potentially modify as part of their response.
