EVALUATION FACTORS AND SCOPE OF SERVICES

SCOPE OF SERVICES
The City of Fort Worth (City) is requesting quotes from qualified firms to establish a contract for scantrons for the Water Department per descriptions and specifications listed in this RFQt.
The City of Fort Worth, Water Department uses Scantron iNSIGHT 4 scanners with ScanTools Plus with Real Time Optical Character Recognition (RTOCR) software to read Liquid Waste Manifest book forms to charge waste hauling companies for dumping liquid waste.
All items supplied resulting from this bid shall be of recent production, unused, and suitable for their intended purpose.
1.0 PROPOSER REQUIREMENTS - Proposer shall:
i. Have and operate a full-time, permanent business address with the ability to be reached by email and telephone.
2.0 General Requirements
Vendor shall provide scantrons according to the following specifications on an as needed-basis:
Liquid Manifest Book
2.1 Page Size: 11 5/8 inch by 8 ½ inch
2.2 Form Type: Multi-Part Cut
2.3 Pages per book: Twenty five (25)
2.4 Paper on Parts 1-5: No Carbon Required (NCR) Paper
2.5 Ink Color on Each Part: White; Yellow; Blue; Green; Goldenrod
2.6 Numbering: Six (6) digit consecutive numbering on each page
2.7 Margins: See attached Exhibit A

2.8 Books shall be printed on Mark Reflex 60# paper, brand name or equivalent.
2.9 All materials shall be Scantron equipment compliant and shall not cause damage to scanner or settings of the scanner.
