About this file

This is a Respondent Questionnaire form issued by the City of Detroit Office of Contracting and Procurement (OCP) for RFP #551406 regarding Fuel Site Inspection & Repair Services. The RFP seeks qualified respondents to provide repair services, labor, and parts for active fuel sites operated by the General Services Department's Fleet Operations, as well as upgrades, improvements, or official closure services for inactive fuel sites and underground storage locations. The questionnaire does not specify response deadlines, site visit requirements, bidder conferences, award dates, or contract term details in this attachment. However, the form was revised as of September 8, 2026, indicating an active procurement process for the City of Detroit in Michigan.

The questionnaire focuses on respondent qualifications and capacity rather than pricing terms, and does not identify any explicit set-asides in this attachment. However, the form requires respondents to disclose whether they would be willing to sign a Priority Hire Agreement with Detroit at Work to prioritize hiring Detroit residents for the project, suggesting an emphasis on local workforce development. Respondents must provide detailed organizational information, including FEIN, years in business, employee counts, and specifically the number of Detroit residents currently employed and those projected for this contract. The form mandates submission of at least three professional references from the last five years, subcontractor information with verification that subcontractors are not debarred, organizational charts, and resumes for key personnel. Additionally, respondents must disclose any claims, lawsuits, terminated contracts, debarred individuals or entities with financial interests, and whether artificial intelligence will be used in the proposed solution.
