About this file

This is a pricing sheet submission template for Workers Compensation Third-Party Administrator Services for the Atlanta Public Schools in Georgia. The pricing proposal requires vendors to submit completed pricing sheets in the specified format covering six primary service categories: Base Administration Fees (offered per claim, per employee, or as a flat fee), Medical Bill Review (per bill), Nurse Case Management (per case), Pharmacy Management (percentage of spend or per script), Legal/Litigation Services (per case or hourly rates for partners, associates, and paralegals), and System/Reporting Fees (flat fee). Additionally, vendors must include Implementation costs as one-time fees and may identify any additional or other fees. The contract term pricing must remain firm for Year 1, with subsequent years subject to escalation caps specified by the vendor, along with a proposed escalation methodology.

Vendors must provide detailed pricing assumptions including estimated annual claim volume, number of employees covered, definitions of medical-only and lost-time claims, and clarification on whether reopened claims and Allocated Loss Adjustment Expenses (ALAE) are charged separately or included in base fees. The pricing submission must include a vendor certification signed by an authorized company representative. Additionally, vendors must disclose all performance guarantees and associated fee credits, including service-level standards for claim handling timeliness and communication, reserve accuracy, medical management performance, litigation management, outcome-based guarantees such as closure ratio and claim duration targets, cost containment guarantees, financial benchmarks, and savings guarantees. A penalty structure must be defined for single and multiple metric failures with specified credit percentages and a maximum total fees at-risk cap.
