About this file

This document is a Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion form for a Lower Tier Covered Transaction, specifically related to the Louisiana Milk and Milk Products Bid for 2025-2026. The certification is required by regulations implementing Executive Order 12549, and is mandatory for any responsive bidder on contracts equaling or exceeding $25,000 or for any audit services contract regardless of amount. Bidders must complete this form to certify that they are not currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in federal transactions.

The form requires bidders to provide a material representation of fact about their current standing in federal transactions, with potential consequences for false or erroneous certifications. Participants must immediately notify the proposing entity if their certification status changes and agree not to enter into lower-tier transactions with debarred or suspended entities. The document outlines specific terms and definitions from the U.S. Department of Agriculture regulations, and emphasizes that participants may determine the eligibility of their principals through various methods, including checking the Nonprocurement List. Failure to comply with these requirements could result in suspension, debarment, or other remedies pursued by the federal government.
